Licensing Act 2003 Personal Licence application guidance
These guidance notes should be read before submitting your application form for a personal licence.
The application form etc should be submitted to the Local Authority in which you liverather than work.
Qualifications
Before you submit an application for a personal licence you should obtain one of the following qualifications. Please note that the accreditation numbers have recently changed but all the qualifications listed below are considered acceptable.
1. BIIAB (QCF) Level 2 Award for Personal Licence Holders QCF Accreditation no 501/1494/3 or
BIIAB (QCA) Level 2 National Certificate for Personal Licence Holders. QCA Accreditation Number: 100/4866/2.
2. EDI (QCF Level 2 Award for Personal Licence Holders QCF Accreditation no 500/9146/3 or
EDI (QCA)Level 2 National Certificate for Personal Licence Holders. QCA Accreditation Number: 100/4865/0.
3.GQAL Level 2 National Certificate for Personal Licence Holders. QCA Accreditation Number: 100/5040/1.
4.HABC (QCF) Level 2 Award for Personal Licence Holders QCF Accreditation Number: 500/9974/7.
or
HABC (QCA) Level 2 Award for Personal Licence Holders QCA Accreditation Number: 500/7383/7.
5.NCFE (QCF) Level 2 Award for Personal Licence Holders QCF Accreditation Number 600/1323/0 or

Basic Disclosure (DBS)
The DBS disclosure form which you submit to the Council must be less than one monthold at the date of submission otherwise your application will be rejected.

Application Process
The following documents should be submitted to the licensing authority at the address below or brought in person to Access Croydon on the ground floor ofBernard Weatherhill House.
1)The completed application form.
2)The original qualification certificate.
3)The original DBSdisclosure form.
4)The completed certificate of disclosure of convictions form.
5) 2 passport sized photographs with a white background, one of the photographs must be certified on the back (see the list of approved signatories for the appropriate wording).
6)A cheque for £37 made payable to Croydon Council.
7)Proof of right to work in the UK (See full list of acceptable proofs on application form)
