1. This Access Agreement enables insert participating authority to access the services sourced by the Contracting Authority (LPP) as intended in the Official Journal of the European Union Contract Notice 2016/S 159-288501

1. LPP’s single point of contact is Jacqueline Eastwood who will be responsible for supporting insert participating authorityin their understanding and operation of the Framework Agreement for the Provision of Outpatients Pharmacy Services

1. LPP will make available to insert participating authorityall details of Framework Agreement for the Provision of Outpatients Pharmacy Services including support documentation as required by insert participating authority

1. insert participating authoritywill be entitled at any time during the term of this agreement to conduct further competitions and award contracts under the framework agreement.

1. Unless otherwise agreed in writing, insert participating authoritywill have full responsibility and ownership for the administration and management of each individual contract awarded subsequent to further competitons which will include, but is not restricted to, the preparation and issue of specifications, the receipt and evaluation of proposals and the issue of service contracts specific to their own business needs.

1. LPP will retain overall responsibility for the management of the Framework Agreement for the Provision of Outpatients Pharmacy ServicesFramework Agreement.

1. Any variation to the terms and conditions of this framework must be notified, in advance, to LPP

1. insert participating authorityshall provide management information to LPP on an annual basis including;

• Total number of contracts awarded by supplier, lot and sub-category
• Total level of business expenditure by supplier, lot and sub-category
• A record of any failures by suppliers to provide products or services in accordance with the relevant order.

Duration and termination

• This agreement shall commence on 2nd January 2017and shall give insert participating authorityaccess until 31st December 2021,the date of expiry of the primary term of the framework agreement..
• LPP will notify insert participating authorityif it intends to extend theFramework Agreement for the Provision of Outpatients Pharmacy Services.
• If LPP extends the term of the framework agreement it is the responsibility of insert participating authorityto request that the access agreement is renewed.

• If this request is not made then insert participating authorityshall not continue to access the framework agreement

LPPreserves the right to withdraw insert participating authorityaccess to the Framework Agreement for the Provision of Outpatients Pharmacy Servicesat any time where it is apparent thatLPP business is being adversely affected by this agreement and/or where the quality of service provided by the suppliers is impacted. In the event of such action being taken, any ongoing current contracts will be honoured.
