Reports to: Living Well Manager

Work Base: East Cornwall.

Salary: £18,065 per annum

Hours of Work: 37 hours per week Monday to Friday

The nature of this post will require travel therefore a requirement of this post will be access to and the use of a reliable car and a current driving licence.

Role of the Living Well Coordinator

The Living Well Coordinator’s prime role is to provide direct support and care coordination for people referred into Living Well.

The Coordinator is required to work collaboratively as part of a value based local integrated health, social care and voluntary sector team, developing innovative ways of working and relationships to improve the coordination and quality of care provided, resulting in improved outcomes for individuals, their carer’s and families.

The Coordinator will be expected to provide support to volunteers and peer team members empowering them to work effectively within the multidisciplinary team, to ensure each person is supported to live the life they wish to, to the best of their abilities, keeping their lifestyle goals at the forefront of their care planning.

The Living Well Manager/Team Leaderwill be part of the local Health and Social Care Multidisciplinary Integrated team, with specific responsibilities to specified GP practices and hospitals.

The Coordinator will have the following responsibilities around leadership, management and providing support to people.

Leadership

  • Uphold the values of Living Well in every aspect of the role.
  • Promote the values of Living Well, positively influencing and challenging attitudes and practice whilst enabling others to adopt the same values.
  • Be responsible for providing information to share peoples stories, information snippets, feedback and photographs, as well as creating opportunities for interested parties to see Living Well in action.
  • Participate in specified activities, for example, task and finish groups, community engagement and research events, following the direction of the Manager/Team Leader.

Team Membership

  • Participate fully in Age UK Cornwall & The Isles of Scilly local team and the associated management processes including 1:1’s and performance conversations.
  • Assist the Manager/ Team Leader to supportand train other team members, using 1:1 meetings, performance conversations and other tools as required.
  • Recruit, support, train and manage volunteers.
  • Support a safe learning environment, where practice can be monitored, challenged, recognised and rewarded by all team members in order to continually improve practice.
  • Develop meaningful productive team relationships with the multidisciplinary staff and volunteers working within specified GP practices and hospital settings, including GPs, nursing, mental health and social care workers.
  • Provide clear robust coordination within the multidisciplinary teams to ensure the persons goals and action plans remains at the forefront of any care planning. This will involve regular consistent attendance and proactive contribution at the specified GP practice and hospital based multidisciplinary team meetings.
  • Adhere to the agreed policies and practice of Age UK Cornwall & The Isles of Scilly and those of the organisations the team are working in.
  • Be responsible for keeping accurate records and provide evaluation data as required.
  • Provide accurate and timely performance information to enable the Manager/Team Leader to provide required reports within the framework agreed by partners.
  • Develop and maintain good working relationships with the wider Age UK Cornwall & The Isles of Scilly team members.

Supporting people

  • Be proactive in receiving referrals, promptly making contact with the referred person to arrange a guided conversation.
  • Have a guided conversation with each person that focuses on them as a whole person, their carer’s, their lifestyles, aspirations, wants and needs. To use this information to co-develop a set of goals and an action plan that the person feels will enhance their sense of well-being and help them to manage their long term health conditions.
  • Work with the person to understand what triggers a crisis situation and what response is required, by whom and when to avert future crisis situations, informing their Anticipatory Care Plan.
  • Inspire and motivate the person to make healthier lifestyle choices by entwining options such as physical activities, healthy eating and long term condition peer support into their action plans.
  • Provide a conversation summary to the referrer and other key workers in the person’s circle of support that shares the person’s goals and action plans.
  • Explore, using local community networks and makers, opportunitiesto help the person to build their own new or existing sustainable social networks, offering choices that support their action planning and goal attainment.
  • Develop opportunities for people, where none exist, ensuring they are sustainable and can operate independently of Age UK Cornwall & The Isles of Scilly.
  • Understand and maintain a good working knowledge of the range of support services on offer e.g. assistive technology, equipment, specialist services e.g. hydration, dementia care, police, social care, therapy, handyperson, health trainers, peer support groups.
  • Promote and support informal and formal volunteering opportunities to members of the public, practitioners and the people who have been referred to Living Well.
  • Develop close working relationships with key local people/community makers and community groups in order to ‘open up’ potential support and activity opportunities for older people in their communities.

General

  • Uphold the highest standards of personal conduct in all matters relating to the role.
  • Maintain current knowledge and practice, attending training and personal development opportunities as required.
  • Maintain an up to date working knowledge and practice of Age UK Cornwall & The Isles of Scilly’s policies and procedures as well as those of the organisations working within Living Well, including NHS Kernow, Peninsular Community Health, Cornwall Partnership Trust and Cornwall Council.
  • Abide by the Organisation’s confidentiality, equal opportunities and other relevant policies.
  • Follow set procedures for maximum efficiency and to work within health and safety regulations and accepted guidelines.
  • Undertake other duties which are consistent with the duties and responsibilities of the post as required by the Age UK Cornwall & The Isles of Scilly’s Living Well Services Manager.

To fulfil the requirements of this role the Living Well Coordinator must be prepared to gain the following knowledge and/or experience.

  • Maintain a working knowledge of the Governmental legislation/policies that are relevant to the role e.g. Care Act, 5 Year Forward plan.
