SOCIAL JUSTICE STUDIES

SCHOLARSHIP ACCEPTANCE FORM

The Social Justice Studies ScholarshipCommittee has established the following requirements and terms for all award recipients. Please read the following carefully, before signing the document:

1. I have officially declared my college major to be Social Justice Studies. My major must continue to be Social Justice Studiesfor scholarship renewal.

1. I must maintain a minimum GPA of 3.0 in my major and an overall cumulative GPA of 2.5 to receive renewal of my scholarship.

1. I must maintain full-time undergraduate status during the Fall and Spring terms to receive and to renew the scholarship.

1. I understand that the Scholarship is for the amount of $1,500 per year.

1. I understand that the Scholarship is renewable for up to 3 semesters after the initial award (2 academic years)

6. As a recipient of the SJS Scholarship, I recognize that I should be a person of strong character and integrity. Therefore, this scholarship may be terminated for conduct, which in the discretion of the SJS Scholarship Committee, would tend to bring the School, College or University into disrepute. Such conduct would include, but not be limited to, academic dishonesty, such as plagiarism and cheating. It may also include conduct that would result in the individual being placed on social or university probation.

8. Renewal of scholarships is contingent upon continued external funding. Students must also meet all eligibility criteria to be considered for renewal. Students who fail to meet eligibility requirements will be placed on probationary status. No more than one probationary period will be allowed.

9. The SJS Scholarship Committee reserves the right to review and revise scholarship program requirements.

The Committee will be the determining body for scholarship renewal or revocation.

I declare that I understand the standards for receiving and the renewal of the Social Justice Studies Scholarship, and certify that I agree to abide by the terms and conditions as described in this document. In signing this form I also understand that the SJS Scholarship Committee, or the Office of the Dean of the College of Justice & Safety, will contact the EKU Office of Judicial Affairs to note any infractions that I may have committed hereafter.

I hereby authorize Eastern Kentucky University to release information about this award to local, state and nationwide media. I further agree to cooperate with the EKU Office of Public Affairs and to provide additional information or releases as may be appropriate.
