BEFORE FILLING IN THIS COMMON APPLICATION FORM, it is recommended that you read all of the booklet Admission to School, Academic Year 2018/2019 – Advice to Parents.
The closing date for receiving applications is 31 October 2017.
For children resident in BarnsleyThis common application form is to enable you to express preferences for three Barnsley and/or any other Authority school(s) and rank your preferences in order. There is also space on the application form to give reasons for your preferences.
If your child attends a Barnsley primary school, you must return the common application form to the Headteacher of your child’s current school by the closing date 31 October 2017.
If your child attends a school outside Barnsley, you must return the common application form to the
Admissions Officer, People Directorate, PO Box 634, Barnsley S70 9GG by the closing date.

supplementary information form. Return this form directly to your child’s Primary school or the
Admissions Officer – see address details above, if your child attends a school outside Barnsley.
For voluntary aided schools in other Authorities – contact the Local Authority.

If a child is offered a place at a preferred school on the basis of false or intentionally misleading information provided by you, then the offer of the school place will be withdrawn. If your child is attending the preferred school having secured the place on the basis of false or intentionally misleading information the sibling criterion will cease to apply in the event of you making an application to the same school on behalf of a younger child in your family.
