To apply for ONE or ALL of the scholarships listed below, fill out ONE Standard Application Form
and make photo copies for EACH scholarship you are applying for, filling in the title of the scholarship
and returning it to the address indicated. Also, attach a copy of your TRANSCRIPT and DATA
SHEET plus any other requests indicated for EACH scholarship you are applying to.

ANDREW P. HAWRYLUK SCHOLARSHIP FUND – Open to seniors with a preference to fire-
fighters relatives and or extended families. Indicate in Section H of the application your relationship to
a firefighter such as grandfather, father, brother, etc. Open to Essex County residents.
Return to Mrs. Perry in the Guidance Department.
BARBARA GOLDBERG MEMORIAL SCHOLARSHIP – Must be planning to attend a two or
four year college and be pursuing a business related career. School activities, community involvement
and academics will be are factors. Attach a one-page essay describing the following:
Why did you choose your field of interest, goals you hope to accomplish in your field of choice, your
plan of action to reach these goals and how the scholarship will help to accomplish these goals. Must

BEVERLY EDUCATIONAL FOUNDATION BOOK AWARD – Applicant must have pursued
rigorous educational course work, been involved in school life at Beverly High School and has

BEVERLY MOTHER’S CLUB SCHOLARSHIP – Candidate must be a senior at BHS in good standing and who is going to pursue a post secondary education in the fall. Must exemplify the spirit of volunteerism through school, community, religious, and or civic organizations. Please enclose a short essay describing how your mother or a female caregiver has influenced you and the development of your character. BEVERLY HOSPITAL NURSES ALUMNAE ASSOCIATION SCHOLARSHIP – Open to a
graduating Beverly High School Senior who will be pursuing a career in nursing. Academics and extra- curricular activities will be factors. Please enclose a short essay on why you are planning to pursue a
nursing career. Also attach a letter of acceptance from the college Applications should be dropped off at Guidance with Mrs Perry.
BHS CLASS of 1969 BOOK FUND AWARD – Senior must show academic progress frim freshman year
Through senior year. Not based on financial need. BHS Seniors only. Application should be dropped off at
Guidance with Mrs. Perry.
BHS PTSO SPIRIT OF VOLUNTEERISM BOOK AWARD – Candidate must be a senior at BHS
in good standing and who is going to pursue a post-secondary education in the fall. Must exemplify the

BEVERLY KIWANIS CLUB SCHOLARSHIP – Open to deserving seniors furthering their education and who show need, achievement and are involved in activities. Open to Beverly Residents. Return to: Mrs. Perry in the Guidance Department.
BEVERLY ODD FELLOWS AND SUMMIT ENCAMPMENT SCHOLARSHIP- Must have above average grades, participate in community service and have financial need. TWO scholarships given.

BEVERLY TEACHERS ASSOCIATION SCHOLARSHIP – To a senior dependent of a BTA
member or a deceased member. Based on scholarship, activities and financial need. Return to:

BLUE VASE MARKETING SCHOLARSHIP - Senior must be majoring in management,
marketing or advertising, have financial need and be involved in extra-curricular activities and

BRISCOE MIDDLE SCHOOL PTO ACHIEVEMENT AWARD – Must have graduated from
Briscoe Middle School and be graduating from Beverly High School. Also scholastic achievement
and community service will be factors. Not based on financial need. Fill in sections A through F

BRUCE STALKER MEMORIAL SCHOLARSHIP – Open to seniors with average grades and who have financial need. Athletic particiaption preferred. Return to Mrs. Perry in the Guidance Department.

CENTERVILLE IMPROVEMENT SOCIETY SCHOLARSHIP – Student must live in Centerville
and have a parent be a member of the Centerville Improvement Society. Based on character, need and
other factors. Must be accepted at an accredited school. See Special Application Box (in Guidance)
for an additional supplemental sheet. Return to: Mrs. Perry in the Guidance Department.
CENTERVILLE SCHOOL PTO SCHOLARSHIP – Senior must have graduated from the
Centerville Elementary School. A personal cover letter, one reference must be included Section H must be completed One reference letter section H must be filled out. Return to: Mrs. Perry in the Guidance Department. Beverly High School Seniors Only.
COMMUNITY SCHOLARSHIP FUND OF ROCKPORT -Must be a graduating student from Beverly High School pursuing a Bachelors or Associates Degree in Business or Finance at an accredited institution.. Merit or needs based. Two scholarships will be awarded, male and female. Must be a BHS senior and Beverly resident Return to: Mrs. Perry in the Guidance Department
COVE SCHOOL PTO AWARD – Applicant must have graduated from the Cove Elementary School
and be continuing his or her education. Please attach one letter of recommendation and the financial

DR. SAMUEL P. CARITO DDS SCHOLARSHIP – Must be accepted to a Dental Hygiene or Dental Assisting Program. Candidates must provide proof of acceptance and a short essay, less than 500 words, written statement explaining reasons why they want to be Dental Professionals. Must be a Beverly High

ELISABETH BELKNAP SCHOLARSHIP - Must be attending a four year college and be majoring in business. Also must be involved in extra curricular activities and community involvement and have need.

GEORGE F. ROBERTSON MEMORIAL SCHOLARSHIP – Open to seniors with average grades
and real financial need. Children of Beverly Police Separtment given preference.

HANNAH ELEMENTARY SCHOOL BOOK AWARD - Senior must have attended the Hannah
Elementary School and be accepted at an institution of higher learning. Also, candidate must have
community involvement, good academics and citizenship. Include a cover letter stating your education

HERMAN L. NELSON SCHOLARSHIP– Be a child or grandchild of a member in good standing of the Herman A. Spear Post #331. Children and grandchildren of deceased members also eligible. Non members must have parent, grandparent or legal guardian who was honorably discharged from the armed forces. Need is also a factor. FOUR scholarships given. JAMES L VITALE MEMORIAL SCHOLARSHIP – Senior must be furthering their education and
show need and academics. Must also be involved in athletics preferably hockey. Beverly High School

JOAN PLOSZAY MEMORIAL SCHOLARSHIP – Applicant must demonstrate an understanding
And a commitment to civic responsibility, demonstrate participation in community service activities for

JOHN T. BROWN MEMORIAL SCHOLARSHIP - Senior must be currently in the music program
at Beverly High School. An essay must be included on what or how music has influenced your life.

NORTH BEVERLY ELEMENTARY SCHOOL PTO SCHOLARSHIP – Must have attended the
North Beverly Elementary School for a minimum of two years and be a Beverly High School Senior who resides in Beverly,in good academic standing,community involvement and has financial need. NORTHEAST REGIONAL AMBULANCE SERVICE, INC. SCHOLARSHIP – Must have a career objective to work in the healthcare or public safety, e.g. Emergency Medical Services, fire Services, Law Enforcement, Nursing, Physician, Physician Assistant, Nursing Assistant.

FIREFIGHTER PAUL JAMES ROBERTS MEMORIAL SCHOLARSHIP – Open to seniors with a preference to firefighters relatives and or extended families. ROBERT KERR MEMORIAL SCHOLARSHIP – Open to seniors who have participated in sports, preferably basketball. Character will also be a factor. Preference wll be given to those individuals who will be majoring in Criminal Justice. Return to: Mrs. Perry in the Guidance Department

SUSAN A. DODGE SCHOLARSHIP FUND – Open to graduating seniors that have been accepted at and plan to attend Colby College, Brown University or Bowdoin College. Open also to undergraduate
students already attending named college. A letter of acceptance and a copy of your financial aid package and FAFSA must be provided. Two letters of reference and a copy of your transcript are also required. High scholastics, need and character will be factors. Return to: Mrs. Perry in the

TEAM MCKEOWN SCHOLARSHIP – Senior must have attended the McKeown Elementary School and have shown outstanding commitment to their community. Share ways in which you have participated in community building at school, in the city of Beverly, or in the greater local or national community. Please enclose a one page letter that details your accomplishments and plans for the future and explain why you would be the ideal candidate to receive the TEAM McKeown Scholarship. Scholarship will go to seniors who are trying to make a difference in the world they live in. THE TIARA FOUNDATION SOCIETY SCHOLARSHIP – The Tiara Foundation Society is a non-profit organization that raises funds for a scholarship to benefit a single parent annually which will help continue a college or trade school education or technical school No are requirement to apply. 2. Must be enrolled or accepted to a trade school, technical school or college.3. Must have earned a 2.3 grade point average or greater and show official proof of GPA.4. Must submit one teacher/advisor recommendation from said institution or copy of acceptance letter. 6.
