Office Policies for Dental Partners of Towson & Owings Mills

We are committed to providing the best possible dental care; to do so, there are certain guidelines that our office must follow. Please read the following very carefully.

Financial Policy

As a courtesy, we will submit claims to our patients’ insurance carrier on their behalf. However, it is our responsibility to collect all estimated co-payments on the day of service. Please understand that the co-pays are estimates based on the accuracy of the information available to us from the insurance company. Additional payments may be due after a claim is submitted.

We are unable to carry balances unpaid by your insurance carrier for more than 90 days after the initial submission of a claim. After 90 days, all patients are required to pay their balances in full and seek reimbursement from your insurance carrier. We reserve the right to pursue all delinquent accounts via a third party collection agency or attorney.

Finally, please familiarize yourself with your dental benefits. It is your responsibility to be aware of your deductibles, time restraints, yearly maximums, and financially accountability.

Treatment Options and Informed Consent

We recommend most patients have routine dental exams and cleanings every six months to maintain good dental health. X-rays are taken once a year to detect decay and signs of gum and bone disease. Minors (18 years and under) will receive fluoride treatments during their exam and cleaning appointments to help prevent tooth decay.

It is our obligation to offer all treatment options. Sometimes these options may require referring patients to trusted colleagues and/or dental specialist who possess the requisite experience or discipline to treat such conditions. These decisions will be made on a case-by-case basis.

With any treatment, there is always a risk of failure. These risks will be discussed with the patient prior to treatment. All patients will be required to sign a consent form prior to treatment. The patient may refuse to sign the form, and the practitioner may choose not to perform any treatment.

The demand for dentistry that is undetectable to the casual observer is continually growing. We will routinely place non-metal restorations (white composite) fillings. While this is one of the most conservative and esthetic options, it may insure a higher co-pay, so please familiarize yourself with your dental benefits regarding restorative treatment.

Treatment of Minors

All patients under the age of 18 must be accompanied by a parent or legal guardian to their appointments. If not, they must be accompanied by an adult with a letter of treatment consent, signed by the parent or legal guardian with a contact phone number. Otherwise, no treatment will be rendered that day.

Broken and Missed Appointments

We reserve the right to charge for broken or missed appointments without a 24 hour notice. A fee of $37.00 per appointment for a scheduled appointment block may be assessed. If you fail three consecutive appointments in one calendar year we reserve the right to discontinue all services. Please inform your provider before your appointment if you have any questions or concerns.

I have read and understand the terms above.
