Forms received after November 18th will NOT be included in the lottery process!

Individual Checks for each class must be included with completed registration forms in order to be eligible for the lottery. Please be sure to write separate checks for each activity for each child. In the memo section, please specify for which class the check is written. Also, if your last name is different than your child’s, please note that on the check as well. Please make checks payable to Sherwood PTO.

Registration Procedures

Class Dates:

For the Winter/Spring session, Kindergarten lunch bunch classes will be offered on Thursdays and Fridays, and after school classes will be offered on Mondays, Wednesdays, Thursdays and Fridays. Classes will run from January9th– June2nd. Class dates differ slightly; please see the schedule grid for specific class dates. There will be no enrichment classes on days when school is a half-day, closed, cancelled, early dismissal, or when there are special events at the school.

NOTE: The winter and spring sessions are combined for 2017, so the classes will run for one long session with enrollment in November.

To Register:

Registration Deadline is FRIDAY, NOVEMBER 18th, 2016.

You will be notified via email to confirm your child’s placement in each class no later than Friday, December 9, 2016.

●Registration forms: Please complete ALL forms, except if your student was enrolled in fall enrichment, then you only need to complete the registration form and transportation form.

●One registration form per child (please write legibly in black or blue ink).

●One emergency form per child

●A separate check for each class

Drops & Refunds

A class may be dropped for a full refund before the first class occurs. If a student's participation is terminated due to poor behavior, a refund will NOT be given.

Angel Fund

Angel Fund is available for families who need assistance with tuition. Please contact Mr. Walker or Grace directly for further information.

Payment Policy

●To be included in the lottery, a separate check for each class for each child is required. DO NOT write one check for multiple classes or children.

●Checks should be made payable to “Sherwood PTO”. Write the child’s name and which class they are registering for in the memo section of each check.

●If your child has a different last name than that on the check, please specify the child’s name in the memo section.

●Any classes for which your child registered but did not get into, your check will be voided and destroyed. If you want your check returned to you, please call Jenny Kim at (847) 579-0277, , promptly.

There will be no refunds given once the classes have begun.

Emergency & Allergy Forms

We must have an emergency form on file for each child registered. The emergency forms will be kept in a master binder accessible to all enrichment teachers and to the enrichment supervisor on duty.

Please complete the “Allergy/Additional Information” form if your child has any food allergies and/or other information the teacher must know.

Lottery

If you sign up for multiple classes on one single day, you must rank them in order of preference. Classes will be filled first by the students who selected that course as their first choice. In the event that the registration for a specific class exceeds the maximum allowed for that class, a lottery will be held. If that same class is offered in the following session, any child who did not get into the class will be given first priority next session. Also note the maximum number of classes desired in the event he/she gets into all of them. Again, make sure you write a separate check for each class. We cannot accommodate friendship or carpool requests.

PTO Behavior Policy

The PTO reserves the right to terminate the participation of any student in the enrichment program for disruptive behavior, or if the PTO or program supervisors consider the continued participation of the student to interfere with or disrupt the ability of the staff to conduct or manage the activities of the program. Further, all students are reminded not to touch the personal belongings of the students and teachers whose classrooms they are using for enrichment. Failure to comply with this rule may result in the student being terminated from the program. A refund will not be issued if a child's participation is terminated due to a behavior issue.

Attendance

There will be a supervisor present for the duration of the classes. He/She will monitor the attendance in the classes.

If your child will not be at his/her after school enrichment class, please call the school office by 3:00 on that day to notify the supervisor of your child’s absence for that day. You must also notify your child’s classroom teacher so that he/she knows to send your child home and not to the enrichment class.

Bus Transportation

A bus will be available following the enrichment classes at 4:15 to take the children home. It is not available for any class that runs past 4:15. If your child normally qualifies to ride the school bus, they will be eligible to ride the after school activity bus. The bus routes will be established after registration so that the stop closest to your house can be determined. Please complete the After School Activity Transportation Form.

Please be aware that bus routes may run late, as there is only one bus available for all students.

Noggin Builders:

Build problem solving and creative thinking skills through fun, hands on activities in STEM (science, technology, engineering and math). Making slime, exploring animal habitats, discovering outer space, and experimenting with surfacing tension are examples of some of our captivating themes. (Min 6/Max 14)

Yoga Bash Mind Body Fun:

Partner poses, self-awareness and respect for others are emphasized in this fun and creative class. Children will learn a variety of techniques and exercises to help them relax and calm their minds, as well as build strength, increase flexibility and improve coordination. Kids will practice challenging yoga poses by imitating animals and elements of nature, Self expression is highly encouraged as we create meaningful art, and make up our own yoga stories to act out. (Min 5/Max 15)

After School Enrichment Class Descriptions:

Mad Science, Grades 1-3

Mad Science is a class filled with fun, spectacular, hands-on science experiments! Some of the cool, new, hands-on science topics will include Glow Show, PhPhactor, Lab Works and more. Each young scientist will be able to make some fun take home projects too, including a Blacklight Writer, Graduated Gear Kit, Atomic Coins and many others. All materials are included and instructors are provided by Mad Science of Northern Illinois. (Min 10/Max 20)

After School Sports, Grades K-3

Every week the kids will get the opportunity to use different pieces of gym equipment that they may have been introduced to in class, but don’t get to use on a regular basis. Scooters will continued to be offered as part of this class, designed by Mr. J and taught by him or his assistants! (Min 8/Max 20)

Flag Football, Grades 1-4:

Taught by members of the little giant coaching staff. Focus will be on offensive and defensive strategies in game settings. Fundamental skill development will be emphasized in a supportive and nurturing environment. (Min 6/Max 16)

NEW! Amazing Arts by Sunshine Crafts, Grades 1-5

Amazing Arts combines all your favorite art projects and mixed media in this unique class. Kids will make tye dye shirts (they will wear art smocks for this session), experience pottery painting, paint fired ceramic arts, paint a canvas acrylic painting, do recycled art projects and more. Lessons will include concepts and techniques kids can use for a lifetime and the experience of creating awesome projects will result in great self esteem and a sense of accomplishment. All new lessons for every session.(Min 6/Max 20)

NEW! Noggin Builders, Grades 2-3

S.T.E.M. Explorers

Explore hands-on, real-world projects in S.T.E.M. (Science, Technology, Engineering & Math) while challenging your problem solving and creative thinking with fun, hands-on activities. Design a roller coaster, make chemical reactions, investigate the bounce of a ball, build your own “flinker” or create a balloon-powered car. These are just some of the challenges we will dig into in S.T.E.M. Explorers. (Min 6/Max 14)

Play Hard Hoops, Grades 1-3

Play Hard Hoops philosophy is to teach the fundamentals of basketball in an encouraging environment where players of all ages and ability can learn, improve and have fun. The class will teach the skills of dribbling, passing, shooting, defense and basketball footwork while incorporating skill-based contests and controlled game play to teach age-appropriate game concepts. Play Hard Hoops currently runs programs in over 10 locations and works with almost two thousand kids throughout Chicago. (Min 8/Max 16)

Ninja Warriors, Grades 3-5

Have you ever thought about climbing the playground a different way than the traditional way? What about climbing the swing set and scaling across the top? How about walking on top of the hand rails around Sherwood or jumping through trees? If this all sounds like a fun... Come spend Mondays after school in a class designed by Mr. J (and taught by him or his assistants) as we will experiment and climb all around Sherwood! Let's see how many Ninjas we have at Sherwood School. Come climb with me! (Min 8/Max 20)

NEW! Ninja Warriors, Grades 1-2

Come spend Fridays after school in a class designed by Mr. J (and taught by him or his assistants) as we will experiment and climb all around Sherwood, with a concentration on fitness obstacle courses that involve climbing and jumping! Let's see how many Ninjas we have at Sherwood School. Come climb with me! (Min 8/Max 20)

Crafting with Kim, Grades 1-5:

Taught by local artist and teacher, Kim Bloomberg, this class will include a variety of materials including duct tape, polymer clay, sculpting, mosaics and more! Using these materials, children are encouraged to use imaginative minds and creativity to express their thoughts artistically. (Min 5/Max 20)

Stage Stars, Grades 1-4

This is the perfect place for the young performer who is looking for a fun filled class to let their talents shine. Harness the acting tools you already possess – your body, voice, and imagination – to physically express ideas, transform into multi-dimensional characters, and tell moving stories. Gain skills in creative thinking and public speaking, building confidence in yourself and your craft. Students choose current pop hits and musical theatre songs to develop an original musical using their own creative ideas to help write the play! They will receive a speaking role, sing and dance in the final production. Skills explored during this class will be script writing, singing, dancing, and acting. The session culminates with a final performance that will be performed on stage in the Multi purpose room. (Min 6/Max 15)

NEW! After School Cooking, with iCook! Grades K-5

iCOOK is a really fun, hands-on after-school cooking program for children. Our mission is simple: to help teach children to love everything about food – cooking, eating delicious and healthful dishes (of course!) and learning about nutrition. We make every experience fun because we start with great recipes and fresh ingredients, colorful cooking stations and engaging activities. We also teach basic table manners and dining etiquette, and of course, clean-up skills. Although ingredients will be purchased with our best efforts to avoid nuts, this class is not advised for those students with food allergies as some items may be cooked in facilities with nuts.(Min 5/Max 20)

NEW! Yoga Bash Mind Body Fun, Grades K-5

Chess Scholars, Grades K-5:

No previous knowledge of chess is necessary. Each class will consist of a fun interactive teaching period and guided practice time. Both new and returning participants are welcome and will get to the next level under the guidance of an experienced Chess Scholars coach. There will also be a chess competition with prizes and a pizza party at the end of the session! Each child will receive a participation award and a chess keychain. (Min 6)
