Community Case-manager II

Purpose:

This Case-Manager position provides Case-management, BST, and PSR services to clients within the Community Department and or within the AVMH STEPS Program orPACT service delivery model that is also rehabilitative in nature. The focus of this position is client based to ensure the clients are receiving the approved services they are approved for.

Specific Job Duties:

  • Provide Case-management, BST, PSR, assessment, and supportive services to clients assigned to as specified by Medicaid Services Manual Ch. 400.
  • Coordinatecare & services with multidisciplinary treatment team.
  • Write and implement Rehabilitation Plans for clients.
  • Maintain an active liaison with family members/ advocates or significant others, communicates on a regular basis with other program staff to assure consistency in training, treatment and facilitation of the integrated Individual Program Plan.
  • Complete and submit all DAP, Case-management, Assessments, and Progress notes on all clients directly worked with.
  • Regularly attend all internal Treatment Team Meetings for clients.
  • Be able to problem solve day-to-day problems that may arise.
  • To coordinate care with third party agencies that client may be involved with such as other mental health agencies, medical groups, Primary Care Physicians etc.
  • Identifies the health needs of the patient and the expectations of the family.
  • Makes an assessment of the resources and support network at their disposal.
  • Seeks to ensure the continuity of care by avoiding fragmentation and duplication of services.
  • Assist with the Prior Authorization process up to and including preparation of FA11-A forms for submission.
  • Additional assignments/ projects may be assigned to this position but will be within the scope of practice for a QMHA status with NV Medicaid.

Candidate Qualifications:

  • Must have a Bachelor’s Degree or higher in a human related field or hold an AA Degree in a human related field with at least 4 years of relevant experience; and be actively enrolled with Medicaid as a QMHA or higher.
  • Must have at least 1 years’ experience in working with persons with SED or SMI diagnosis.
  • Must have adequate writing skills in a clinical setting.
  • Must have independent and reliable transportation and be able to provide proof of insurance that includes the coverage of additional passengers.
  • Must be computer literate with an ability to use Microsoft Word, Excel, and Adobe.
  • Must have a valid driver’s license.
  • Must have a current CPR card.
  • Must have a current TB test (within 2 years).
  • Past case-management experience is preferred but not required.
  • Must be willing and available to work some nights and weekends.
