·  When my circumstances change such that I am no longer entitled to claim Free School Meals for my children, I will advise Erith School immediately.

·  I understand that if I continue to claim Free School Meals when my circumstances change I will be liable to repay the cost of meals taken by my child(ren).

To qualify for Free School Meals please see below:

1. Providing Free School Meals

One complete hot or cold meal with a drink to a specified value (£2.00) per day for students under the age of 19 years at 1 September.

2. Who can claim?

To qualify for Free School Meals, parents must be in receipt of at least one of the following support payments:

·  Income Support; or,

·  Income based Employment and Support Allowance; or

·  Income based Job Seekers Allowance; or

·  Child Tax Credit but not Working Tax Credit and have an annual income of £16,190 or below from 6 April 2010 as assessed by Her Majesty’s Revenue and Customs. With regard to Tax Credits, Her Majesty’s Revenue and Customs guidelines are used for qualification and definition of household income; or

·  Guarantee Element of State Pension Credit

·  Support under Part VI of the Immigration and Asylum Act 1999

Families in receipt of Working Tax Credit or other support payments, apart from those mentioned above, are not entitled to Free School Meals.

Children who receive Income Support or Income based Job Seekers Allowance in their own right are also eligible to receive Free School Meals.

Children who qualify for Free School Meals, based upon the above criteria, must also receive education both before and after the lunch period in order to be eligible.

3. Providing evidence to support your claim:

Income Support: You will be required to confirm entitlement to Income Support.

Child Benefit: You will be required to confirm entitlement to Child Benefit. Acceptable forms of Child Benefit evidence are:

Her Majesty’s Revenue and Customs/Child Benefit Office letter: a copy of the whole letter, which is less than 1 year old and which states your name, address and child/children’s name(s); or

Tax Child Credit Award Letter: a copy of the whole letter, which is less than 1 year old and which states your name, address and child/children’s name(s); or

Other evidence (e.g. court order): a copy of the whole letter providing that you are the legal parent/carer responsible for the child/children.

If the necessary evidence is not included with your application, this will delay the processing of your application, the form may be returned to you and no Free School Meal award will be made. Therefore please assist us by enclosing the correct documentation.

4. How to apply:

Complete the Application Form and return, marked for the attention of Mrs. Evans, to Erith School, Avenue Road, Erith, Kent, DA8 3BN.

5. What happens when you apply?

The Free School Meals administrator will write and let you know whether you qualify and if you do:

(a) You will be issued with a free meals authorisation to have effect from the start of term, or the following Monday if an application is received during a term.

6. Can I obtain help in making my application?

Yes, if you need help you can telephone to discuss your application or to make an appointment with Mrs. Evans between 10.00 am and 11.30 am from Monday to Friday. Please ensure you bring along any documents in support of your application.

7.
