Warranty for SINs A-3 (a, b), A-4 (a, b), A-6, A-13c, A-26 (a, b, c): Quaisar Enterprises, LLC dba Health Products For You, warrants and implies that the items delivered hereunder are merchantable, fit for use for the particular purpose and will be free from defects in material and workmanship for a period of not less than one (1) year from date of acceptance and in conjunction with contractor’s extended warranties. (See Attachment for warranty time frame.) Quaisar Enterprises, LLC dba Health Products For You, will replace, at its options, all parts found defective within the period with cost of replacement, including shipping charge, to be borne by Quaisar Enterprises, LLC dba Health Products For You. Under no circumstances will any equipment covered by this guarantee be returned without (a) advance written notice to Quaisar Enterprises, LLC dba Health Products For You, or (b) obtaining shipping instructions from Quaisar Enterprises, LLC dba Health Products For You.
Return Goods Policyfor SINsA-3 (a, b), A-4 (a, b), A-6, A-13c, A-26 (a, b, c):If a shipping error occurs due to Contractor’s error, the Contractor will pay freight both ways for return/ exchange of goods. If an ordering facility error occurs, the ordering facility will pay return freight. No restocking fee will apply either.Custom orders are non-returnable. In addition, for Contractor to accept return of items, items must be returned in original container(s) and unused within 45 days from date of acceptance. Please call the contractor’s customer service department for a return authorization number (RA#) at 1-866-316-0162 x 201.

Disaster Recovery Participation for SINs A-3 (a, b), A-4 (a, b), A-6, A-13 (c), A-26 (a, b, c): Quaisar Enterprises LLC dbs Health Products For You is willing to participate in Disaster Recovery Program.
Tracking Customer for SINs A-3 (a, b), A-4 (a, b), A-6, A-13 (c), A-26 (a, b, c):TrackingCustomer andMost Favored Customer, BethelHealthcareCenter, is the identified tracking customer. If during the course of this contract, for any sales under the Maximum Order, the net price of any awarded item is reduced to BethelHealthcareCenter, then, the Government net pricewill be reduced proportionately.
