Hours of Work21 hours per week (3 days). Some out of hours workwill be necessary for which Time of in Lieu may be taken

Job PurposeThe post holder will provide information, advice and advocacy to Carers in Wandsworth through 1:1 support and group work.

The post holder will facilitate access for Carers in Wandsworth to appropriate support services, both statutory and voluntary. This will involve the direct provision of services, collaborative work with other local agencies, outreach work and training.

Principle Tasks:

1.0Service Provision

1.1 To participate in the support and information service duty system

1.2To provide information and support to Carers by appointment at the office, through home/community visits and on the phone

1.3To provide outreach surgeries at different locations within the borough and to work across the Carers’ Centre offices in Wandsworth and Balham

1.4To support and empower Carers to influence local service delivery

1.5To provide training and information seminars for Carers, community leaders and local voluntary and statutory service providers

1.6In conjunction with other staff to ensure that Wandsworth Carers’ Centre services are accessible to Carers in Wandsworth

2.0Service Planning and Development

2.1 To establish and maintain contact with appropriate statutory and voluntary agencies, community groups and Carers living in Wandsworth

2.2To represent Wandsworth Carers’ Centre at appropriate meetings seminars and committees

2.3To assess and keep under review the emotional and practical support needs of Carers living in Wandsworth

2.4To appraise the quality, appropriateness and accessibility of local services for Carers

2.5To develop plans to identify 'hidden' Carers and to maintain the profile of the Carers’ Centre within Wandsworth

2.6To provide advice to statutory authorities on their initiatives for Carers

2.7To develop and implement appropriate service plans for the project in light of assessed need, and services available form other voluntary and statutory service providers

2.8To ensure that the role of Carers Support and Development Officer is clearly defined, understood and valued by staff, volunteers and other agencies

2.9To fund raise for services as required

2.10 To complete regular Carers Star support plans with Carers

2.11To support Carers with long term planning, including developing action plans with Carers

2.12To have up to date knowledge of welfare benefits and maximise the income of Carers through accessing eligible benefits.

3.0Staff Responsibilities

3.1To work as part of the team at the Centre to ensure quality service provision for all Carers

3.2To actively participate in team meetings as required

3.3To actively participate in management supervision

4.0Quality, Monitoring and Reporting

4.1To ensure the services provide high quality and accurateadvice and support

4.2To record and monitor work within the established procedures of the Centre

4.3To produce regular written reports

4.4Produce quarterly monitoring reports

5.0Policies and Procedures

5.1Participate in the production of policies and procedures of the Centre

5.2Carry out the above duties with due regard at all times to the Equality & Diversity, Confidentiality and all other policies of Wandsworth Carers’ Centre

6.0Other Duties

6.1To participate in training as required

6.2To keep abreast of developments in the field of Carers

6.3To undertake other duties as appropriate to the grade, as negotiated with the Chief Executive
