PURPOSE

The Facility Namemust be prepared for a variety of disasters and emergency situations both natural and man-made. In order to react to these disasters and emergencies the Facility Nameshall prepare and plan for the responses and reactions required in these situations. The Hazard Vulnerability Analysis (HVA) assesses the impact of potential emergencies on the hospital and identifies those factors that must be addressed when preparing the Emergency Operations Plan (EOP).

POLICY

Facility Namewill identify the potential hazards, threats, and adverse events, and then assess the impact on the care, treatment, and services that must sustain during an emergency. The Hazard Vulnerability Analysis (HVA) is designed to assist in gaining a realistic understanding of the facility’s vulnerabilities, and to help focus the resources and planning efforts. The community’s and region’s HVA assessments should also be considered the assessment for the organization. A list of priority concerns will be developed from the HVA, will be used for planning purposes, and will be evaluated for accuracy at least annually. The HVA will include evaluation of the ability to provide services, the likelihood of events occurring, and the consequences of those events.

PROCEDURE

The Emergency Management Committee will review for additional hazard analysis anytime throughout the year when new hazards or situations have been discovered within the Facility Nameand/or community.

This analysis will be documented on the HVA Assessment Tool. The priority (probability, severity, etc) and impact of each of the disasters or emergency situations will be evaluated using the HVA Worksheet. A score ranging from 0 – 4, with 4 being the most severe or with the highest impact, will be assigned to each probability and impact factor.

The final step is to revise or develop an additional Emergency Response Plan for each emergency identified in the HVA.
