PLAAFP Development Schedule Matrix Guide

This form is intended to be completed by the IEP team prior to the IEP to provide a framework for developing the PLAFFP.

This form can also be used as an observation form to assist in determining the student’s current performance within educational environments.

In this column, list the student’s daily schedule including all primary activities, classes and/or transitions. / This column can be used to identify the general instructional outcomes for all students in the classroom as well as the target student that may / may not be included on the IEP. For example, during “arrival,” the instructional goals for ALL students may include independently taking off outerwear, getting materials ready, and taking a seat. For the target student, the outcome may be independently taking off outerwear with assistance required for the other general instructional outcomes. This allows for PLAAFP discussion about what expectations for all students the target student is able to perform or needs supports / goals to learn to do so. / IEP goals and/or objectives directly from the IEP should be written in this column based on when/where they were addressed in the student’s schedule. This will allow the IEP team to discuss progress on the IEP goals relative to the environment in which they were addressed. / List in this column, all the supplementary aides and services used to support the student in making progress on goals and objectives, meeting instructional outcomes, navigating the environment, and participating in instruction as independently as possible. These include but are not limited to:
