As a result of bid response to KUMC RFQ #683F11-08, the parties enter into a six month contract for the provision of products and services enumerated herein. Renewal terms may be entered into as mutually agreed to. The initial term shall be September 15, 2011 through March 14, 2012.

KUMC anticipates procuring the following animal feed over the next six months. However, this volume is merely an estimate, and may increase or decrease, based upon agency requirements. The successful vendor shall enter into a six month contract with the Medical Center, with the potential of four additional six month term extensions, if/as mutually agreed upon. Substitutions are permitted only with KUMC approval. Please provide pricing of items below.

1.  Vendor shall insure that a sufficient amount of all animal feed listed below will be available to ship to KUMC within 7 days ARO. All orders shall be shipped FOB destination, prepaid and allowed, clearly marked with the purchase order number. If delays in delivery are anticipated, the vendor shall immediately notify the ordering agency of the revised delivery date or partial delivery date. The order may be canceled if delivery time is unsatisfactory. The vendor shall inform the agency of any supply or delivery problems. Continued delivery problems may result in termination of the contract for cause.

2.  Presently, many State Agencies use a State of Kansas Business Procurement Card (Visa) in lieu of a state warrant to pay for some of its purchases. No additional charges are allowed for using the card. Please indicate if you accept the Business Procurement Card for payment.

4.  At the time of renewal, prices as bid may remain at the price bid or may be based on a price adjustment, either upward or downward, keyed to industry and changes. Vender shall furnish figures at least thirty days prior to expiration date to substantiate any claim for increase. Price increases shall not exceed five percent of the existing contract. If parties to the contract cannot agree on renewal terms, it is hereby understood that the contract will be re-bid.
