Instructions:

1.  Hold a commissioning and training meeting with owner maintenance personnel representatives to turn over the system

2.  Review items below and have attendees initial each item acknowledging understanding and acceptance of training/documentation

3.  Video record the entire training and provide a copy to owner for future reference or training needs. Submit a copy to PDI for record

4.  Have meeting attendees sign/date the bottom as acceptance of responsibility for the system going forward

5.  Submit a completed copy of this form to and CC meeting attendees and GC for record

8.  Acknowledgement: End user is responsible for backing up and maintaining all tenant data
