POSITION VACANCY NOTICE

Job Title:Records Management Clerk

Site:595 W Main St, Watertown

Report to: Operational Supervisor

Status: Non-exempt

Hours: 40 hours per week, Monday through Friday(some evening hours as needed)

Travel: Limited travel and attendance at various Credo meetings/training programs required. Travel to CredoCommunity Center sites.

Description:

Provide a full array of administrative support functions to Credo Community Center Outpatient Services

Essential Job Functions:

  • Responsible for patient records management (Creates, maintains, closes and archives files; QA patient records).
  • Reviews patient records for completeness, accuracy and ensures compliance with OASAS, OMH, HIPAA and MEDICAID/Medicare regulations.
  • Retrieves patient records and refiles charts in proper sequence.
  • Check and mail all counselor correspondence, making sure proper releases accompany each piece of correspondence mailed or faxed.
  • Prepares and provides service delivery reports.
  • Answer phone and records messages; dispense basic agency information and route calls to staff members.
  • Responsible for Outpatient Services correspondence (i.e. proof read drafts, finalizes documents, mails document, etc.).
  • Maintains the office forms and files and establishes new files and revises existing ones in accordance with policies and procedures.
  • Cross trains other staff.
  • Conducts data input in a variety of tracking systems.
  • Serves as Outpatient Operational Support Staff upon request.
  • Ensure the daily clinical documentation is complete and accurate in accordance with OASAS, OMH,and Medicaid/Medicare regulations.
  • Keep statistics regarding clinical services and best practices for client care.
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives, and safety, environmental, and/or infection control standards.
  • Support co-workers
  • Performs other duties as assigned

Minimum Job Qualifications:

  • High School Diploma
  • At least 2 years of experience that is directly related to the duties and responsibilities specified.
  • Knowledge of administrative and clerical procedures and terminology.
  • Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
