As you may be aware, the new South Glasgow University Hospital and new Royal Hospital for Sick Children are due to open on the current Southern site early in 2015.

With this in mind, please note that positions based within the Victoria Infirmary, Mansionhouse Unit, the Western Infirmary and the current Royal Hospital for Sick Children at Yorkhill will change location and move to the new hospitals.

Gartnavel General Hospital and Glasgow Royal Infirmary will also have some services affected by moves to the new Hospitals.

SUMMARY INFORMATION RELATING TO THIS POSITION

This post will provide general medical services to the Adult Eating Disorder service within NHS Greater Glasgow & Clyde. The post will be based in Florence Street Resource Centre and you will work as part of the Eating Disorder Service to provide physical health screening and care for new and current patients of the service supported by other team members.

Greater Glasgow and Clyde Adult Eating Disorder Service is a multidisciplinary service which sees patients with moderate to severe eating disorders across Greater Glasgow and Clyde. Patients access the service from their local Community Mental Health Team (CMHT). They continue to receive Psychiatric care from their CMHT during their contact with the AEDS. The AEDS offers a psychologically focussed intervention ranging in intensity from outpatient appointments at the base or in the community, to attendance at the AEDS day programme. The service has four specialist beds for AEDS patients on Armadale Ward, McKinnon House, Stobhill Hospital. The Consultant Psychiatrist in the AEDS is medically responsible for the inpatient care of these patients.

The service sits under the South Glasgow management structure (Calum MacLeod, Head of Mental Health, South West Glasgow). It is operationally managed by Clive Travers, Service Manager, Mental Health South and clinically led by Dr Charlotte Nevison, Consultant Clinical Psychologist.

Patients with Eating Disorders have a high risk of medical complications of their illness. The current medical monitoring of patients who attend the AEDS lies with individual General Practitioners and the AEDS recommends the level of medical monitoring according to current guidelines (NICE 2004, QIS 2006). This development post allows the AEDS to monitor patients’ physical health within the service by introducing physical monitoring of the patients at highest risk in the service.

The monitoring will include a general physical examination of all new patients entering the service, including weight, height and body mass index, a range of blood tests and regular ECGs. The Medical Officer will produce a report on the physical state of all new patients and will undertake regular medical monitoring and review of all patients in the service. The current caseload averages 50 patients, with approximately 70 referrals per year and an average of 6 new referrals per month. While it is likely that most of this work will take place at the service base, Florence Street resource Centre, occasional home visits may be required. It is proposed that the Medical Officer runs a Physical Health Clinic at the start of every week, supported by a Band 6 senior Eating Disorder Practitioner. Patients will be invited to attend this Clinic on assessment and as required. The MO will review all the investigation results with the Consultant Psychiatrist who will supervise the doctor and will share the results at the Team meeting on a Wednesday morning so that treatment options can be considered further. The MO will keep in touch with the patients’ General Practitioners regularly by letter and phone. When required to do so, the MO will liaise with other medical staff in Psychiatry and General Medicine to agree ongoing treatment across services. This may include occasional attendance at case reviews or Care Programme Approach meetings. It is expected that the post will comprise three sessions per week, one for the Clinic at the start of the week, one and a half for the Team meetings and other meetings and half a session for Supporting Professional Activities relating to the post. The MO will contribute to the design of this post and operational procedures for it, and will be involved in teaching and service development as appropriate and agreed.

The post holder will be provided with a thorough induction into the service, and will have supervision, CPD and appraisal. This post is an opportunity for GPs or doctors with other relevant experience to work in a multidisciplinary Psychologically-led specialist Eating Disorder Team and develop specialist medical experience in this field.

2. JOB PURPOSE

The Medical Officer will work as part of the Adult Eating Disorder Service to provide physical health screening and care for new and current patients of the service, supported by other Team members.

3. DIMENSIONS

The two main diagnostic groups of Eating Disorders, Anorexia Nervosa and Bulimia Nervosa, usually develop in young people. Most patients are female but at least 10% are male. They have high levels of morbidity and mortality and the risks associated with these illnesses are often hidden from patients, their families and the Medical profession. The risk of death is the highest of any psychiatric condition and these illnesses often cause lifelong ill health such as osteoporosis, gastrointestinal and endocrine abnormalities and cardiac problems.

Some patients referred to the service have been patients of the Child and Adolescent Mental Health Services but most of the referrals arise de novo in the Adult services where they are referred from the local CMHTs.

The Adult Eating Disorder Service came into existence in 2008 as a psychological community service to the population of Greater Glasgow and Clyde. Currently based at Florence Street Resource Centre, it is a large multi-professional Team with Psychology, Dietetics, Nursing and Occupation Therapy represented. The service is led by the Consultant Clinical Psychologist. The Consultant Psychiatry input is to the inpatient service which began in 2010 with 4 beds based in a 20 General Adult Psychiatric Admission ward. The ward is supported by additional AEDS nursing staff who provide cover for meals and support Monday to Friday 7.30 am to 7pm. The Resource Centre provides the Team base and space for the outpatient and day patient services. Medical input is being sought for the Team base where the Medical Officer will be supervised by and responsible to the Consultant Psychiatrist in the Team.

4. ROLE OF MEDICAL OFFICER

Core tasks include:

·  To undertake a full physical examination to include neurological examination of all patients when they are referred to the AEDS

·  To undertake a range of blood tests on all new patients to the service, to include U&Es, LFTS, glucose, bicarbonate, calcium and corrected calcium, magnesium, selenium, zinc, phosphate, cholesterols, FBC, B12 and folate

·  To undertake an ECG on all new patients

·  To review all results and discuss with the Consultant Psychiatrist on the Team, highlighting any abnormalities and concerns

·  To arrange appointments for repeat blood tests and EGCs as required clinically, after discussion with the Consultant Psychiatrist and the AEDS Team members and to undertake these tests as arranged for all the patients who attend the service

·  Although these examinations and investigations will almost always take place in the Team base, occasionally the MO may be required to visit other areas to undertake these tasks, including patients’ homes, with support

·  To attend the AEDS Team meeting on a Wednesday morning to discuss patient care and clinical investigation results and to agree treatment plans with other Team members. This plans may require that the MO liaise with other services, GPs and medical staff and that the MO arrange further investigations and follow-up for patients in the AEDS

·  Occasionally, to arrange emergency assessment by other services and specialists based on the results obtained and after discussion with the Consultant Psychiatrist in the Team

·  To produce a report on the physical state of all AEDS Team patients on a regular basis

·  Routine prescribing of medication is not required

·  To maintain regular contact with patients’ GPs by letter and/or phone

·  To attend Team development meetings, regular CPD, to contribute to care governance as part of professional development (audit, incident reporting etc)

·  To attend appropriate teaching and training and participate in regular Appraisal and Revalidation

·  To undertake occasional case reviews as required with Team/Consultant Psychiatrist

·  No out of hours duties required

The management of cases is guided by various National and local policies, guidelines and protocols within an ethical framework.

The Medical Officer will be a core member of:

·  Local Team and clinical meetings

·  Medical Officer meetings

·  Medical Officer’s CPD Programme

Once experienced in the medical aspects of these cases, the MO will have the opportunity to support and supervise GP Practices who manage Eating Disorder patients. There will be opportunities to become involved in teaching and training.

5. ASSIGNMENT & REVIEW OF WORK

The Medical Officer will be managed clinically by and report to the Consultant Psychiatrist. The Team Lead is the Consultant Clinical Psychologist. The Team is managed by the South Glasgow Management Team led by the Head of Mental Health, South West Glasgow. The Clinical Director for South Glasgow is based at Leverndale Hospital.

Appraisal and Revalidation will be managed by GG&C Health Board.
