Parent-Teacher Conference Night

Parent-Teacher Conference Night is soon approaching. This night has been developed to assist parents and teachers in scheduling appointments and in providing for more effective conferences. This process is simple but requires your help to be successful.
Step 1: Review the form below and the directions. Fill in the top section with your student/parent names and a
phone number or e-mail where you can be reached during the day.
Step 2: Complete Part I listing the teacher(s) with whom you want to have a conference.
Step 3: Have yourchild bring the form to each teacher listed or e-mail the teacher directly. The
teacher(s) will complete Part II indicating the time and location of the conference(s) or e-mail you a confirmation.
Step 4: Your son/daughter will return the completed form to you, so that you may bring it with you

The teacher will complete Part II (lower portion of form) and the student will return the form to you. If for any reason, the teacher is unable to meet with you, the teacher will contact you.

The student will contact the teacher(s) indicated above, and after all conferences have been scheduled, return it to parent(s). All conferences should be scheduled by Thursday, December 3rd, at the latest.
