DISPOSITION:Original is maintained in the district office for three (3) years and a copy is to be sent to Professional Standards at Headquarters by 1/15 for the previous year

NOTES:Separate line entries will be made each time the evidence area is accessed. If more than one case number is addressed per access to the room, the agent and supervisor must document each case number separately and sign each line separately. If an agent is removing only a portion of the evidence associated with a case, the removed portion of the evidence should be detailed in the comments section. On AL-50t should be maintained for each location where temporaryevidence is stored. One AL-50t can be used for multiple storage areas at the same address.
Completion Instructions:
- District: District where the evidence room is located
- Site Name: Put the name of the temporary evidence storage site.
- Activity Date: Month, day and year the evidence storage area is being accessed.
- Activity Time: Time the agent enters the evidence storage room. Do not use military time.
- Agent Signature: Agents signature when exiting the area. No call signs, initials, or ID numbers will be accepted.
- Locker Number: If the same AL-50t is being used for multiple storage containers at the same address, put the number of the container.
- Evidence In/Out: “In” if you are adding evidence to the evidence room. “Out” if you are removing evidence from the evidence room.
- Case Number: Case number associated with the evidence you are in the area to address.
- Seizure Date: The date the evidence was seized.
- Evidence Description/Comments/Destination: Include any additional relevant information regarding the evidence of the specific numbered case. If the item is a sensitive item, put details if possible. (Ex: Firearms- include the serial number, US currency – include the dollar amount, etc.) If only a portion of the evidence per case number is being added or removed, include item numbers. When evidence is being removed, indicate where you will be taking the evidence.
