Requirements for the Availment of the Dole Livelihood Grant

REQUIREMENTS FOR THE AVAILMENT OF THE DOLE LIVELIHOOD GRANT

( per COA Circular 2007-001 and DO 36-02)

For NGOs/POs/cooperative/union/rural workers associations/informal sector organization:

[ ] Certificate of Registration with the Department of Labor and Employment (DOLE), the Securities and Exchange Commission (SEC), the Cooperative and Development Authority (CDA) or other appropriate regulatory agency.

[ ] Authenticated or Certified true copy of the latest Articles of Incorporation or Articles of Cooperation as the case maybe, showing the original incorporators/organizers;

[ ] Project Proposal supported by a Feasibility Study to include:

objectives of the project

project management committee names and individual responsibilities

project partners, if any

list of intended beneficiaries and their addresses

project duration and target dates of completion

detailed project costs including the equity of the organization in monetary terms

other potential or actual sources of project funds

accomplished individual Profile of member/beneficiaries (DKIS blank form attached)

[ ] Financial statements for the three (3) years immediately preceding the application ofproposal

For organization with less than three (3) years of operation

existing bank account

certification from LGU or regulatory agency concerned attesting to the credibility and capability of the applicant’s officers and staff;

[ ] List and/or photographs of similar projects the applicant or proponent is implementing

or has previously undertaken, including sources of funds;

[]Appropriate board resolution (including name of authorized signatory to the MOA), attested and duly subscribed and sworn to by the appropriate officer; to include name of authorized signatory to the MOA

[ ] Photocopy of Certificate of Accreditation as ACP;

[ ] Disclosure by the proponent of other related business, if any, and extent of ownership therein;

[ ] Sworn affidavit of the Secretary of the proponent that none of the incorporators, organizers, directors or officials is an agent of or related by consanguinity or affinity up to the fourth civil degree to the officials of the government office authorized to process and/or approve the proposal.

[ ] Photo copy of Official Receipt (OR) of the organization

For GO/ Local Government Unit/Government Agency:

[ ]Project Proposal and Feasibility Study same as above contents but supported byapplicable existing and duly-approved local development plan;

[ ] Photocopy of the Certificate of Accreditation as ACP;

[ ] Appropriate board resolutionapproving the application for assistance as described in the project proposal, and specifically stating that the LGU or GO has appropriated or allocated funds to comply with the equity requirements.
