PRETRANSPLANT:
1. Able to describe the transplant evaluation process for transplant candidate, including, but not limited to: / Date Reviewed / Transplant Staff Mentor Initials / Comments
·  Medical / surgical evaluation
·  Psychosocial evaluation
·  Educational assessment
·  Laboratory & Radiological evaluation
2. Observes if able, or discusses selection committee and can locate Policy and Procedures on the Intranet and able to describe absolute and relative contraindications to transplantation
3. Observes pretransplant education process and can describe:
a.  Living donation options; related, unrelated, paired exchange
b.  Recipient Evaluation and waiting list process
c.  Transplant Regulations
d.  Organ allocation and Transplant Process
e.  Post-operative phase including immunosuppressive therapy and effects.
f.  Candidate & caregiver responsibility in the transplant process.

TRANSPLANT EVENT:
1. Identifies and responds to educational and psychosocial needs of family/legal guardian during recipient transplant surgery
2. Attend Rounds and describe role of multidisciplinary team in care planning and discharge planning.
3. Educates recipient/family/legal guardian regarding care and responsibilities after transplantation:
a.  Vital signs
b.  Diet and activity
c.  Frequency of follow-up visits and lab studies
d.  Reporting abnormalities to transplant center/primary care provider
e.  Signs and symptoms of rejection/infection
f.  Medications (purpose, dosage, frequency and side effects)
g.  Explanation of procedures required for follow-up (heart biopsies, etc.).
i.  Need for periodic follow-up at transplant center, and
h.  Importance of recipient/family/legal guardian’s commitment to transplant medical and medication regimen.
POST TRANSPLANT
1. Observes Outpatient Care Coordination, Clinic Activities and observes, monitors, assesses and reports to appropriate healthcare provider the recipient’s:
a.  Physical and emotional status
b.  Short and long-term complications of immunosuppression
c.  Drug toxicity, and
d.  Infectious complications
2. Reinforces education and follow-up requirements necessary for successful outcome.
