This memo addresses updates needed to the documents notebooks:

Updated Material for Notebooks:

1)  PFS Form – Participant Final Status from. This is a new form, designed to replaces the PCS- Participant Change of Status, which never worked correctly for the study, and which hadn’t really been used.

2)  PFS QxQs for new form.

3)  DTF form – Periodontal Treatment Form has been updated to fix a typo under local anesthetics.

4)  Current PCS form is no longer a study data collection form.

5)  Form Table of Contents for Notebook (updated).

6)  MOP Chapter 11 – updated to include procedures for the new PSF form

7)  MOP Chapter 7 - Updated to include the questions suggested for the post treatment follow-up visit or telephone call. Questions are included in questionnaire form at back of chapter.

8)  MOP Table of Contents for Notebook (updated).

Please update your notebooks with the materials listed above, and make sure all your center’s staff is notified of the changes. Thank-you.

As discussed in the PC conference calls. All participants must have a PFS form. Sites will need to complete this form for all participants who have already completed the study. The CC can provide you with a list of study ID’s on participants who have had postpartum data entered if this would be helpful.
