The Real Choice Partnership is aimed at assessing the issues that hinder choices for persons with disabilities and long-term illnesses, and implementing a strategic plan to provide for “real choice” in everyday living. Through a two-year grant awarded to United Way of Palm Beach County by the State of Florida and the Governors ADA Working Group, the Real Choice Partnership addresses quality of care and system reforms issues such as accessible and affordable housing, transportation, employment, education, respite and caregiver issues, and meaningful daytime activities as they relate to persons with disabilities or long-term illnesses.

The Real Choice Partnership is currently being considered for a Nursing Home Transition Project funded, in part, by the State. The United Way of Palm BeachCounty match funds would provide for the initial assessment of individuals in institutional settings who would like to return to community life.
