Valentine Express Order Form

Beginning January 27th and going through February 11th, the Student Council will take orders for Valentine Express. If you would like to send something to your child, family member, or special friend, please fill out the form below and return it to the school with the correct amount of money in a sealed envelope. Please mark all mail orders as “Valentine Express.” (THINK GREEN: More than one order may be placed per page. You may use the back and just write out what you would like and whom it is for from.)

_____ $.50 – Kiss-a-Gram – A note, a kiss stamp and a baggie with Hershey Kisses.

The note reads, “You’ve been kissed by ______.”

(Please note that the label will only hold two names for kissed by. It is not possible to put additional messages)

_____ $2.50 – Singing Telegram – A delightful song, note, and a piece of candy telling who it is from.
