Intent to Provide Maximum Treatment Notice

A public water system(PWS) that chooses to provide the maximum level of treatment for Cryptosporidium applicable to their plant type (rather than start source water monitoring), must submit written notification no later than the date the PWS is otherwise required to submit a sampling schedule for monitoring. Alternatively, a PWS may choose to stop sampling at any point after the system has initiated monitoring if the system provides written notification that the system will provide this level of treatment. Submitting this form satisfies the requirement to submit a notice of intent to provide maximum treatment. A PWS must also install and operate technologies to provide this level of treatment by the applicable treatment compliance date.
