THIS NOTICE DESCRIBES HOW PERSONALLY IDENTIFIABLE INFORMATION, INCLUDING PERSONAL HEALTH INFORMATION, MAY BE USED AND DISCLOSED. PLEASE REVIEW IT CAREFULLY.

IF YOU DO NOT AGREE TO THESE PRIVACY TERMS, DO NOT USE THIS SITE OR THE SERVICE.

1. Introduction

MeU CARE (MeU) websites, mobile applications, and services (the "Site") are hereinafter referred to collectively throughout this Privacy Notice as "we", "our" or "us." This Site allows you to access certain health communication related services (collectively, the "Service"), including multimedia communication platforms that give you the ability to share personal health information with people and services. With the Service, you can share information entered by you or people selected by you, such as caregivers or health professionals, with friends or family members, healthcare professionals, caregivers - all as chosen by you.MEU CARE IS A PRIVACY AND SECURITY ENHANCED COMMUNICATIONS PLATFORM. HOWEVER IT IS CRITICAL THAT YOU UNDERSTAND AND AGREE THAT YOU ARE CHOOSING TO SHARE YOUR PERSONAL INFORMATION, AND THAT THERE IS ALWAYS A RISK THAT YOUR PRIVACY COULD BE AFFECTED.

This Privacy Notice explains the collection and use of personal information and personal health information about you and the choices available to you about how your information can be shared, accessed and updated.

By visiting the Site or using the Service, you are accepting the practices described in this Privacy Notice and expressly consent to our collection, storage, use and disclosure of personal information about you as described in this Privacy Notice and as may be directed by you on the Site. This Privacy Notice, your use of the Site or the Service, and any dispute related to use of information provided to us, is subject to the then-current version of the Terms of Use.

In order to use the Site and our Service, you must logon in accordance with the logon procedures for this Site. If you choose not to logon, you will not be able to access the Site or use the Service.

2. Collection of Personal Information

In providing the Service to you, we will collect the following types of information.

Contact Information

The first time you sign in to the Service, the Service may you to create an account. To create an account, you must provide personal contact information such as name, e-mail address, phone number, mailing information, and country/region.

Billing and Payment Information

For certain services, such as for subscription, health sessions and live video communication and monitoring services, we may collect credit card or payment account information which we maintain in encrypted form on secure servers. We may also collect financial information such as bank account, insurance and tracking numbers.

Personal Health Information

We may also collect and store health information about you. Here are examples of the types of health information we may gather:

Information You Give Us – All personal health information you give us. Examples of the types of information you may provide us include: measurements, such as weight, blood pressure or glucose levels; lab results; medications; health history, and other health or medical information (such as prescription information).

Information Accessed through Third-Party Data Services. When you use our Service, you are may access health-related information about you that is stored with third party-data services and such information will then be available to us.

Third-Party Information – Health-related information about you received from third-parties (such as nurses, doctors, health plans, medication history data providers or family members and caregivers.)

Unique Identifiers, such as account numbers and passwords, health plan account numbers, electronic personal health record account numbers.

Demographic Information, such as age, education, gender, and zip code.

3. Cookies and Tracking Information

Cookies

We use cookies with this Service to:

(1) enable you to sign in and to help personalize the Service; (2) provide you with customized content related to your account; (3) monitor and analyze Site usage; (4) remember you when you return to the Site; and (5) conduct research to improve the Site or Service. A cookie is a small text file that a Web page server places on your hard disk. The Site may also contain hyperlinks to third party web pages or social media sites. If you click on those links, those third party web pages or sites may also place cookies on your computer when you visit those sites. You should review the privacy practices of those sites with respect to cookies. The use of cookies by our tracking utility company and service providers is not covered by our privacy notice. We do not have access or control over these cookies. Our tracking utility company and service providers use session ID cookies to provide service to us or to make it easier for you to navigate our site. You have the ability to accept or decline cookies. Check the "Help" menu of your browser or look in your mobile device settings to learn how to change your cookie preferences. If you choose to decline all cookies, you may not be able to use interactive features of the Service or other Web sites that depend on cookies.

Web Beacons

Site pages may contain electronic images known as Web beacons sometimes called single-pixel gifs that may be used: to assist in delivering cookies on our sites, to enable us to count users who have visited those pages, or to deliver other services. We may include Web beacons in promotional e-mail messages or in other communications in order to determine whether you opened or acted upon those messages. The Company may also employ Web beacons from third parties to help us compile aggregated statistics and determine the effectiveness of our promotional campaigns. We may collect information about your visit, including the pages you view, the links you click, and other actions taken in connection with the Service. We do not tie the information gathered to our Customers' or Users' personal information.

Automated Information

Our servers may also automatically collect information, such as Internet Protocol (IP) addresses, browser type, pages visited, multimedia recorded, edited, viewed and shared, access times, duration of visit, referring URL, platform, links accessed, timestamp and exit page information ("Automated Information") with respect to each visitor to the Site, whether or not such visitor is a logged-on user.

Do Not Track

We do not currently use a mechanism to act upon "Do Not Track" instructions from your web browser.

By Facebook if you connect to MeU CARE through Facebook

You may have the opportunity to connect to MeU CARE through Facebook's API when you register for MeU CARE or after you have registered for MeU CARE. If you connect to MeU CARE through Facebook, either when you register or after you have registered, we will collect, store, and use in accordance with this Privacy Policy any and all information you agreed that Facebook could provide to meU through its API. Your agreement (and our access to your information) takes place when you instruct, accept or allow Facebook to register you for a MeU CARE account or otherwise connect to meU through Facebook. The information MeU CARE may access, collect and store may include the following, among other information, as allowed by you, Facebook's API, and your Facebook privacy settings:

Your name

Your profile picture

Your email address

Your gender

Your birthday

Your location (i.e. city)

The names and pictures of your Facebook friends

Your interests and affinity networks

Other information you make publicly available via Facebook

WE DO NOT SHARE THE CONTENT OF (PHOTOS, VIDEOS ETC) NOR THE ACTUAL MEU MESSAGES TO FACEBOOK – INSTEAD THEY ARE HOSTED BY AND VIEWED THROUGH MEU CARE’S OWN PRIVATE SERVERS AND PLAYERS

4. Accuracy of Information You Provide

You are responsible for ensuring the accuracy of the information we collect from you. Inaccurate information will affect the use and effectiveness of the Service and our ability to contact you.

5. How We Use and Disclose Information

Generally

When you create an account, you control who can access the information in the account. If you choose to, you can share your information with others. We use information collected through the Service, including personal information and personal health information, to provide the Service and as described in this Privacy Notice. We will share personal information provided by you with third parties only as directed by you or your permitted invitees or in the ways that are described in this Privacy Notice. We do not sell or rent your personal information to third parties. Information collected through the Service is stored in the United States.

To Provide and Improve Our Services

We use information you provide us and information we obtain from other sources to provide the Service. We also use the information that you provide for such purposes as responding to your requests, customizing future browsing for you, improving our Service, and communicating with you. Depending on the health monitoring services selected by you or your health care professional, we may combine your information with information we receive from other companies (such as demographic data or prescription interaction information) to deliver, improve and personalize our services to you or your professional. We may also collect information for auditing, research and analysis in order to maintain, protect and improve our Service.

To Track Site Traffic Information

We use IP addresses and other Automated Information collected only to track aggregate information regarding Site traffic and usage. One of the uses for which this data is gathered is for statistical purposes so that we may analyze trends and performance over time.

To Protect Us or Third-Parties

We may access and/or disclose your personal information to protect and defend our rights or property (including the enforcement of our agreements) or to act in urgent circumstances to protect the personal safety and welfare of users of the Site or members of the public.

For Data De-Identification and Aggregation

We reserve the right to remove those elements of data that might be used to associate data with any individual, or to "de-identify" the data we collect, and use and share such de-identified data for our business purposes. Aggregated, de-identified data may also be used for health trend analysis, disease control, in assessing the effectiveness of various health programs and for other data analytics purposes. Depending on the circumstances, we may or may not charge third parties for this aggregated de-identified data. By using the Site, you grant us the right to accept payment or other remuneration for such aggregated, de-identified data. We require parties with whom we share aggregated, de-identified data to agree that they will not attempt to make this information personally identifiable. Once the information we collect is in de-identified form, it is no longer subject to this Privacy Notice.

For Use with Third Party Service Providers

In some instances, we use third party service providers to provide services on our behalf, such as server companies, customer service companies, billing companies, data translation and integration firms, and other providers as needed to perform the Service. We require such third party service providers to maintain the confidentiality of any information they receive.

To Comply with Law or for Other Legal or Business Purposes

We may need to disclose certain information to comply with a legal requirement, such as a law, regulation, court order, subpoena, search warrant, in the course of a legal proceeding or in response to a law enforcement agency request. We may also disclose your information as otherwise necessary with your direction or consent (such as through reminder services you may use on the Site). In addition, in the event that our companies, the Company or the product or service offering you have subscribed to through the Site or substantially all of the assets of either company are acquired by a third party, your personal information may be one of the transferred assets. Finally, we may share your information within the meU group of companies for performance of the Service and in conducting meU’s business unless prohibited by law.

6. Sharing Your Information

If you grant access to your information to other individuals, Web sites, mobile devices, social networks, health plans, hospitals, doctors, nurses, caregivers or other health care professionals, you understand that these other individuals, Web sites, mobile devices, social networks, health plans, hospitals, doctors, nurses, caregivers or other health care professionals can access your information, store a copy of it and share it with others. Any such copies of your information are governed by those individuals, Web sites, mobile devices, social networks, health plans, hospitals, doctors, nurses, caregivers or other health care professionals privacy practices. Others may be able to view your information from such copies. We are not responsible for the use of your information by others to whom you or your permitted invitees grant access to your information. You are solely responsible for any unauthorized use of the Site conducted via your password and User ID.

Your MeU CARE multimedia messages may contain personal or private videos, pictures, audio, music or text also referred to herein as ("assets"). Sharing MeU CARE messages and the content within your messages is done so entirely at Your sole direction. Your MeU CARE multimedia messages are shared with others in two ways: (1) internally with other MeU CARE Users directly or (2) or through links embedded in MeU CARE invitations sent by MeU CARE on your behalf to recipients you've selected. It is solely at your risk that you share including making sure that you do not give access to your account credentials or remain in a logged in state on a device that someone else has access to. MeU CARE multimedia messages shared externally are limited-access, where access is gained through an obscured URL system. This obscured URL makes it difficult but not impossible for a third party to guess the URL address of your MeU CARE multimedia message or content without an invitation from you including the relevant URL. However, because the MeU CARE multimedia messages and content are not password-protected, a possibility exists that an unauthorized third party could discover your MeU CARE multimedia messages and/or content. MeU CARE messages shared with others can be designated shareable or "mashable" by You. Assets which comprise your shared or "mashable" MeU CARE messages can be reused, edited and redistributed in part or whole by others, or posted on websites and devices accessible to the public. You take the sole responsibility and risk for sharing mashable messages.

7. Protecting the Confidentiality and Security of Information

We have implemented industry-recognized safeguards to help protect your information from unauthorized access. These safeguards are in compliance with HIPAA as understood by the healthcare industry. These safeguards include:

meU uses SSL (Secure Socket Layer), the industry standard method for computers to communicate securely without risk of manipulation or recipient impersonation. We utilize SSL in the online registration process, in the login process, and in the account management section of the Site.

MeU has received GeoTrust’s highest level of SSL authentication certificate.

MeU uses strong encryption technology to protect sensitive information.

MeU employs firewalls, intrusion detection systems, and system monitoring to protect against unauthorized access of our systems.

Your user accounts are also protected by the password you use to access your online account with meU, and we urge you to take steps to keep your password safe. If you feel your password has been compromised, you should change it immediately by logging into your account and visiting the "My Profile & Settings" section. After you have finished using our Site, you should log out of your MeU account and exit your browser.

WE TAKE THESE PRECAUTIONS IN AN EFFORT TO PROTECT YOUR INFORMATION AGAINST SECURITY BREACHES. HOWEVER, THIS IS NOT A GUARANTEE THAT SUCH INFORMATION MAY NOT BE ACCESSED, DISCLOSED, ALTERED, OR DESTROYED BY BREACH OF SUCH FIREWALLS AND SECURE SERVER SOFTWARE. BY USING OUR SITE, YOU ACKNOWLEDGE THAT YOU UNDERSTAND AND AGREE TO ASSUME THESE RISKS.

8. Account Access, Control and Deletion of Information

You choose whether to create an account with the Service. You control your account and who has access to it. You are responsible for the control of your user names and passwords, and should limit others' access to such information.

You can review and update your account information. You can modify, add, or delete certain personal information, such as your mailing address and email, by accessing your account through the Site. You may also edit certain of your health information. However, if you have shared any of your personal or health information with any third parties, any modifications of your account information by you will not be automatically reflected in any third party copies of your information. If you wish to modify any of your health information held by any third party providers, such as your physicians, you must contact such third parties. We cannot modify such information on your behalf.
