6. BRIEF RESUME OF INTENDED WORK :
6.1 NEED FOR STUDY:
Sthaulya is one of the burning issues globally as it hampers the different systems in the body. An obese person is always prone to land up in complications like dyslipedeamia, hypertension, coronary heart disease, diabetes mellitus, osteoarthritis, impotency as well as psychological disorders like stress, anxiety, and depression. It is not only a condition of the deposition of the fat in areolar tissues but is a condition in which the utilization of the collected fat is hampered.11
Obesity has reached epidemic proportions globally with more than one billion overweight adults; at least three hundred million of them are obese. The emergence of this disease as a public health problem indicates the need of prevention11.
Obesity can be correlated to “Ati Sthaulya” explained in classics under Ashta Nindita Purusha, where Vata and Kapha are involved. Gomutra is considered as auspicious and acts as a Lekhaneeya; thus may have Karshana effect and useful in Sthaulya and Medoroga.5
Gomutra is having the strong pungent odour which makes one refuse to take it. Further the health of the animal, these days, carries a great importance before consuming their products. Many zoonotic diseases are caused by consuming diseased animals & their products. So a thought was put to do such a procedure to remove odour and all the pathogenic agents present in Gomutra without losing its properties. Arka kalpana is one such procedure, which can be adopted for it9 .
Keeping in mind these points, Gomutra Arka is selected for this study to evaluate its efficacy in the prevention of Ati Sthaulya.
6.2 REVIEW OF LITERATURE:
Sthaulya is explained as Santarpanjanya Vyadhi. Ati Sthaulya is included in Asthau-nindita Purusha which is caused by over intake of Guru, Madhura, Shitha, Snigdha Ahara, Avyayama, Diva Svapna, etc.2Chala of Sphik-Shana-Udara, are the important Ati Sthaulya lakshanas2.
As Vata Kapha and Medas are important vitiated factors in Sthaulya, Vata-Kapha and Medahara Ahara is advised.
Deepana-Pchana, Vatahara, Kapha-Mdohara,Tekshna, Ushna, Ruksha Dravyas mentioned in management of Sthauya. Also continuous practice of Karshana has to be done in Sthaulya. Thus, Gomutra which is having Teekshna, Ushna, Ruksha, Deepana-Pachana, Kapha-Vatahara properties is taken for the research.5
Preparation of Gomutra Arka:
The freshly collected Gomutra is transferred in to the distillation apparatus and is continuously heated till 60%. Distillate (Arka) collected should be filtered and preserved in a air tight bottle.9
PREVIOUS RESEARCHES :
About 40 Research works on Sthaulya were conducted.
1) PATIL S.A. (2002) – A comparative study of Triphala with Madhu and Madhoodaka Pana in Sthaulya , Bijapura , Karnataka.
2) SRIKANT SAJJAN (2004) -A clinical study on efficacy of Yavamalaka Choorna in prevention of Sthaulya.Hassan,Karnataka
3) RAVISH S.N. (2005) - A study on the role of Vrikshamla in prevention of Sthaulya. Hassan, Karnataka.
Research conducted on Sthaulya at different research centers, relevant journals, periodicals, magazines will be reviewed.
6.3 OBJECTIVE OF STUDY:
To study the efficacy of Gomutra Arka in prevention of Sthaulya persons in becoming Ati Sthaulya
7 . MATERIALS AND METHODS :
7.1 SOURCE OF DATA:
.30 individuals will be selected for the study, from the OPD and IPD of the S.D.M Ayurveda College and hospital, Hassan
7.2 METHOD OF COLLECTION OF DATA:
30 patients of Sthaulya (over weight) full filling inclusion criteria will be selected for the study.
DIAGNOSTIC CRITERIA:
· Lakshana of Sthaulya as per classics.
· BMI 28-30kg/m2
INCLUSION CRITERIA:
· Patients having BMI 28-30 kg/m2
· Patient of age group 16 to 60 years
EXCLUSION CRITERIA:
· BMI above 30 Kg/m2
· Over weight due to secondary causes and with other systemic diseases like hypothyroidisms, PCOD etc.
STUDY GROUPS:
30 patients of Sthaulya having BMI 28-30 kg/m2 will be randomly divided into the following two groups, each group comprising of 15 patients
1) GOMUTRA ARKA GROUP: Patients of this group will be given 20ml of Gomutra Arka mixed with 40 ml of water, twice daily on empty stomach for two months.
2) CONTROL GROUP: Patients of this group will be advised diet and Vyayama for two months
Dose:
20ml of Gomutra Arka mixed with 40 ml of water will be given twice daily on empty stomach.
Duration of the Study:
Duration of the treatment will be for 2 months and assessment will be done once in a month during treatment.
ASSESSMENT CRITERIA:
1) SUBJECTIVE CRITERIA :
· Decrease in signs and symptoms of Sthaulya.
2) OBJECTIVE CRITERIA :
· BMI
· Waist-Hip ratio
· Circumference of Chest, Abdomen, Hip, Mid am, Mid thigh
· Lipid profile.
The Result will be analyzed statistically through the “t” test
FOLLOW UP STUDY:
After the completion of the trial the patients will be followed up for the period of 1 month at the interval of 15 days.
7.3 INVESTIGATIONS :
Lipid profile will be done before and at the end of the study.
7.4 Has ethical clearance been obtained from the institution? : YES.
