Science Fair Forms

DUE by FRIDAY 9/16/16!Follow these directions exactly! All signatures must be in blue ink!Please be aware that written forms will be accepted however, if students make it on the regional team they will need to be typed. If you would like to type them, you may access the forms at

Form 1: Checklist for Sponsor Adult

  • Fill in: Student’s Name & Project Title (you create) ONLY!
  • EVERYTHING ELSE ON THIS FORM STAYS BLANK (the science fair sponsor will sign off)

Form 1A: Student Checklist

  9. Put your home address and any other address in which you may do the experiment (like a lab)
  10. Instructions for completing the Research Plan can be found in this letter
  11. This will be discussed for students who represent SSMS within the regional team

Form 1B: Approval Form

  • Fill out the top box ONLY – Remember, all signatures in blue ink!

Form 3: Risk

  • You have to come up with something that could be a hazard in your project. Think about things like electrical safety, sharp objects, hazardous materials, clothing/hand/eye protection, etc. Fill in:
  • Student’s Name & Project Title (in pen)
  • Answer questions 1-4 based on possible hazards
  • #5: List an actual source of research that addressed safety
  • Bottom box to be completed by the PARENT if you are doing your project at home

Logbook

DUE by FRIDAY 11/14/16!You need to be keeping a spiral or a composition book for your project. You should start this at the BEGINNING of your project and keep adding to it throughout your project. It must be kept in pen. Any mistakes should be crossed out neatly. Do NOT use whiteout. Additional logbook information is attached to this packet.

Research Plan

DUE by FRIDAY 9/16/16! Your research plan must be typed. It must be set up following this format, in a normal font, font size 12. It can either be emailed to from home, saved in your student file at school, or saved in Mrs. Alberto’s class account. Most of this you have already done, you just need to put it in this format.

  1. Rationale: A brief description about your project that also includes the purpose.

  12. Data Analysis (how you plan to analyze the data)
  13. Bibliography (at least 5 sources you used for your research, and at least 1 should be about safety if you have a hazardous project. Science Fusion (our book) pages R16-17 deal with safety if you need a source.)
  • will help you cite a source as APA, and there is also an APA format guide at the end of this packet.

Science Fair Research Paper

DUE by FRIDAY 11/4/16! This means your project must be COMPLETE by then. This must be a typed document! (12 pt font, single spaced, normal font)Each item is a separate page in your research paper.

  1. Title Page- Include title of project, Student Name and Grade Level (Doesn’t have to follow 12pt rule)
  2. Table of Contents Page
  3. Introduction- Pose the question, purpose of the experiment, and the expected outcomes (hypothesis).
  4. Materials- Listed in bullets (be sure to include exact measurements for everything)
  5. Procedures- should be detailed and numbered.
  6. Data- Display the results of the experiment (Include charts and pictures if possible).
  7. Background research- should have 4 sources, with at least 2 paragraphs for each source (you do not need to write information about your safety source)
  8. Analysis-what do the graphs, tables, and/or pictures show?What aspect would you change if you did your experiment again and why?
  9. Conclusion- brief statement. The experiment supported or did not support the hypothesis and explain why or why not
  10. Bibliography- APA format at least 5 sources one source should be on safety precautions.
