Reimbursement Process**:

1. Complete Reimbursement Form (attached Exhibit A)

2. 3.1.13 - Submit OT/BF form with supporting documentation (agenda, payroll records or time sheet signed by employee and supervisor or OT request signed by employee and supervisor) to:

3. 3.22.13 - An agreement will be provided to your agency for signature

4. 4.12.13 – Return agreement with appropriate signature (address listed above)

5. The agreement will be submitted for the next available County Executive and/or Council’ Meeting (appx. 4 weeks after receipt)

6. Reimbursement will be dispersed after agreements are executed by the County Executive and/or Council. (appx. 3 weeks)

Special Notes: Overtime reimbursement will be allowed for up to five (5) hours. All OT reimbursement requests must be accompanied by submitted department request for OT and PROOF OF PAYMENT to the employee. Banking hours is allowed with proof of time/costs incurred to your city. Reimbursement request documentation must include a copy of the course notice (email announcement) and course agenda. Attendees MUST sign in at the beginning of their class. No reimbursement will allowed if attendee has not signed in. Registering for the course does not constitute proof of attendance.

**Please note that failure or inability to meet the proposed deadlines will result in subsequent delays in reimbursement and may impede eligibility for reimbursement due to performance period and grant guidance restrictions of the grant program.
