(up to $1,000)

Purpose of CMMS Scholarship:

The Caddo Mills Middle School Alumni Scholarship provides financial assistance to high school seniors who exhibit a well-rounded experience in their educational and extracurricular programs. The scholarships are made possible by Caddo Mills Middle School PTO.

Who is eligible for a CMMS Alumni Scholarship?

  • Applicant must have completed grades 6, 7, and 8 at Caddo Mills Middle School.
  • Applicant must be a graduating Senior.
  • Applicant must be involved in extracurricular activities in the community and/or high school as well as when they attended Caddo Mills Middle School.
  • Applicant is planning to continue his/her education beyond high school in an accredited institution of higher education or in a technical school program of at least one year in length.

General Information:

1. The CMMS Alumni Scholarship Committee will consider the following for purposes of awarding scholarships:

  • School Involvement (Extracurricular Activities and Club Memberships)
  • Community Service
  • Achievements
  • Moral Character

2. Scholarships will be awarded at the End-of-Year CMHS Awards Assembly and honored at the 8thgrade award ceremony.

3. Submission deadline will be 9:00 am on Wednesday, April 11, 2018.

Instructions for Submitting the Scholarship Application:

1. Complete all information on the official CMMS Alumni Scholarship application.

2. Please type or print all information. Include your name on all pages, including the essay.

3. ENCLOSE in a sealed envelope:

  • Completed Application
  • Essay –Type a ONE page essay describing how your experiences at CMMS contributed to who you are

today.

  • Attach TWO Recommendation Forms from a non-family member (i.e. neighbor, teacher, employer)
  • Official high school transcript.

4. Submit application packets to Caddo Mills High School office by 9:00 am on Wednesday, April 11, 2018.
