Dear Test Coordinator:

TheSeventh-Day Adventist schools will again use the Iowa Assessments™ Form E, and the Cognitive Abilities Test™ (CogAT®), Form 7, for fall 2017.Conference schools should have all materials necessary to administer the tests.If you need additional materials, please call Naomi Gaul-Treanor at 800.323.9540, ext. 22481 or e-mail .

For budgeting purposes here are the per student prices:

Grades / 2017 Contract Pricing
Grades 3, 5, 6, and 8 (Iowa only) / $12.00 per student
Grades 4 and 7 (Iowa and CogAT) / $14.75 per student
Grades 9 – 12 (Iowaonly) / $12.00 per student

The per student prices above include the following:

Optional achievement/ability testing for grades 3, 5, 6, 8, 9, 10, 11, and 12 are the same price as grades 4 and 7.

Optional achievement and/or ability testing for grades K-2 will be billed at 10% off current catalog prices.

Optional Score Reports:

The score reports listed below can be ordered on page 2 of the Order Form for HMH Scoring Service (OSS) at the time you send your documents in for scoring.

Prices listed for additional score reports represent 2017 pricing and reflect a 10% discount off current catalog prices. Annual price increases for the additional reports occur on January 1 of each year.

GENERAL INFORMATION

To ensure prompt scoring of test booklets and answer documents, it is imperative that you record complete and accurate information.

NOTE: In order for IOWA scores to import to NAD Data Rollup Cumulative Record Report, data must match EXACTLY for: NAD ID number, birthdate and student name. These must match in three places – IOWA Answer Document, NAD Student ID Manager and Student Information System (Jupiter, InformationNow, or RenWeb.). If you have any questions, please contact your conference office.

Filling Out a Student’s Test Booklet/Answer Document:

Students With Barcode Labels:

Follow the barcode placement instructions and complete the Teacher, Grade, School, System/District, and City/State section on each test booklet

Place the barcode label within the four marked corners of the box labeled “Place barcode label in space above” on the answer document.

Write the student’s name on the answer document in the boxes provided to ensure the barcode label matchers the student name. No need to darken any bubbles.

Students Without Barcode Labels:

Be certain the student’s name is spelled correctly and the corresponding bubbles are darkened.

Be sure student’s full name matches name in NAD Student ID Manager and Student Information System

Be certain to write in the student’s ID number right justified (supplied by the North American Division), and darken the corresponding bubbles.

Be certain the correct bubble is darkened for the student’s gender.

Be certain the correct birth date (Month. Day,and Year) is used. CogAT scores are age related. If there is an incorrect birth date or no birth date at all, the student will not receive age-related scores.

  • Be certain birthdate matches birthdate in NAD Student ID Manager and Student Information System.

Completing the Grade/Class Identification Sheet (blue):

Section 1: Information Box

  • Enter September 29, 2017 for Test Date; your school’s name for Building Name; your conference name for System; and the city where your school is located for City.

Section 2: Class Name Box

  • Elementary/Junior High: First and Last name of teacher
  • If the teacher teaches multiple grade levels, and these levels need to be identified, please enter the first and last name of the teacher, then the grade level. For example:
  • JohnRoberts3
  • JohnRoberts4
  • JohnRoberts5
  • Secondary: Freshman, Sophomore, Junior, or Senior

Completing the Grade/Class Identification Sheet (blue) continued:

Section 3: Class CodeBox.

  • Leave blank.

Section 4: Grade Box

  • Darken the circle that corresponds to the relevant grade level. This level should reflect the same grade as the students’ answer documents.

Section 5: Form Box

  • Darken the circle for E on all Grade/Class Identification Sheets.

Section 6: Answer Document Count Box

  • Darken the circle(s) that correspond to the number of student answer documents that are in one grade/class group.

Building Identification Sheet (purple):

  • Use one (1) Building Identification Sheet (purple) for each building in your conference. These identification sheets have been premarked for your convenience. The NAD School ID for each building appears on this sheet. Please make a note of this ID for your files.

Order Form for HMH Scoring Service (blue):

  • Use the special (blue) Order Form for HMH Scoring Service (OSS). This document has a Dataset ID Number in the upper right corner. This number has been assigned to your conference and is used to track your answer documents and identify you as part of the Guam Micronesia program. It is imperative that you use this blue OSS. This OSSmust be placed on top of all documents in box #1. If you misplace this OSS, please immediately contact Naomi Gaul-Treanor by phone: 630.659.1291 or e-mail: . Returning answer documents without this OSS will result in scoring delays.
  • When you are collecting answer documents, be sure that you receive one from each student. Occasionally, a student will insert his or her answer document into the test booklet, and the document never arrives at our scoring service. If you send a document late, you run the risk of the student’s scores not being included in building and conference averages. Late fees may apply.

Shipping Answer Documents:

All test documents must be sent to our scoring center by conference. If we receive individual buildings at our scoring center, we will not be able to process.

  • Stack all answer documents for each teacher by grade. There is no need to alphabetize the documents.
  • Place one (1) Grade/Class Identification Sheet (blue) on top of each teacher group for each building.
  • You must place one (1) Building Identification Sheet (purple) on top of all documents for each building.

Once you have organized your answer documents, you are ready to package them in sturdy boxes. Make sure to pack the boxes securely to avoid shifting of documents in transit. Label each box1 of ___, 2 of ___, etc. (fill in the blanks with the total number of boxes in your shipment). You may return the answer documents in an envelope with your Grade/Class Identification Sheet on top. This will help keep answer documents from tearing. It is recommended that you place all envelopes in a box. Individual envelopes may separate in transit.

Documents will be returned via UPS Return Service. Included in your packet are five (5) UPS RSlabels. Place one (1) label on each box you are shipping to the scoring service. Please refer to the UPS RS Instruction Sheet (goldenrod) for additional directions.
