Risk Assessment Form

This form is to be filled in if hazard ratings/quantities/dilutions make University procedures and/or “Good Laboratory Practice” insufficient control.

Group:
BRC Cambridge NIHR Immunophenotyping Hub / Personnel involved:
Anna Petrunkina, Simon McCallum, Natalia Savinykh, Chris Bowman, Esther Perez, Renato Daniel dos Reis Colaco
Activity being assessed:
Routine fumigation of a cat2 tissue culture cabinet that houses an ariaIII cell sorting machine with Vaporised Hydrogen Peroxide
Hazards identified:
Exposure to cat2 infectious material
Injury from dropping the ariaIII as its loaded onto; or reinstalled on, its custom trolley
Inadvertant interference of the fumigation process
·
Control measures to reduce level of risk
The AriaIII must be prepared ahead of time by running a sample of 10% sodium hypochlorite for 5m, and cleaning the front surfaces of the machine with Klerside-B, followed by water.
All fluidic connections with the fluidics cart should be detached before moving either unit.
The trolley height must be assessed prior to its attachment to the hood, any discrepancy in height, must be corrected by adding or removing spacer elements
The trolley must be firmly attached to the hood with the original manufacturer-supplied bolts. The sorter must be completely supported, before these can be detached.
All staff must vacate the lab when Wolflabs technicians are fumigating the equipment. Staff should only re-enter the lab, when given the all clear by Wolflabs
The ariaIII should be replaced in the hood, using the steps above in reverse order
