A.Prior to Placement on PDR:
Action / Comments / Date Completed- Formal PDR notification letter completed including the following required elements:
- Deficiency/deficiencies identified and documented
- Deficiency/deficiencies linked to ACGME competencies
- Methods for remediation documented in writing
- Remediation outcome measures identified
- Time period (4 months) for remediation documented in writing

B.Notification of PDR:

- Copy of signed PDR letter given to resident at conclusion of meeting
- Copy of signed PDR letter delivered to GME office (within 3 days of meeting)

C.Monitoring of PDR Progress:

- Copy of signed letter to GME office (within 3 days)
- Original letter placed in resident training record
- Unsatisfactory or incomplete PDR – formal letter indicating unsatisfactory performance and one of the following (require prior approval by the DIO):
- Placement on a second PDR period
- Disciplinary action including but not limited to probation, suspension, or dismissal
- Non-reappointment
E.Outcome of PDR:
