Youth Rally to Ocean City, Maryland
January 4 – 6, 2013
THIS TRIP IS FOR ANY STUDENT 6th – 12th GRADE
EACH STUDENT NEEDS a parent/guardian signed ESUMC permission form, medical release form, and conduct covenant. All of these forms are available in the youth room, or on the youth website: www.esumc.net/youth under the “Forms and Info” page.
These papers, and total payment need to be submitted no later than Sunday Dec. 30, 2013.
Packing List

Swim Suit and Towel (The hotel has a pool)
SWIM SUITS: Girls must bring a one piece suit or tankini—no two piece swim suits.
We will be packing the vans on Thursday evening January 3rd from 5:30 – 6:15pm. Please bring your baggage at this time. We will be leaving for the youth rally right after school on Friday, so there will be no time to pack on Friday afternoon. On Friday, you may bring a book bag, but you will need to keep it with you during the van ride to Maryland. If you cannot bring your luggage at the designated time, please arrange an earlier time with Scott.
Payment Information:
Non-Refundable Deposit of $50 due by Sunday December 2.
Total Amount Due: $144 by Sunday December 30.

Your $130 payment covers transportation costs, youth rally registration, a 2 night hotel stay, and all Saturday and Sunday meals. Partial scholarship money is available for any student who needs assistance.
Meals: All meals are covered except for Friday evening dinner. We will be stopping for dinner on the way to the youth rally. You can either bring money or a bagged meal for this stop. All Saturday and Sunday meals are covered in your $144 payment.
Departure: Please arrive at ESUMC, ready to go, by 2:30pm on Friday January 4th. We will leave the parking lot at 2:45pm. We understand that Stroudsburg Middle School does not dismiss until 3:05, however, due to the length of our trip, and the start time of the event, we need to leave by 2:45pm
Return: We will be arriving back at ESUMC around 5:30pm on Sunday January 6th. Students will call or text their parents on the way home to give an exact time of arrival.

Conduct Covenant
Conduct
I will conduct myself in a manner that is acceptable as both a representative of East Stroudsburg United Methodist Church and a follower of Christ.
By signing this form, both youth and parent understand that inappropriate conduct may result in the following:

Loss of privileges
A call home to parents resulting in a pickup from the youth rally.
Courtesy
I will be respectful to the hotel we are staying at by keeping my room in order (no mess) and being courteous to other guests.
General Rules
While on this trip, all youth must stay in a group of 3 or more. This rule is non-negotiable, and failure to adhere to this rule will result in loss of privileges.
1. As a matter of courtesy and safety, always inform an adult volunteer where you are going.
2. All medicines must be turned in to Scott before we leave, with signed instructions from a parent.
3. NO PURPLE ROOMS. At no time will a boy enter a girl’s room; and at no time will a girl enter a boy’s room.
4. Control Yourselves. Potentially dangerous or damaging horseplay along with inappropriate displays of affection will not be tolerated. Adult leaders will determine the boundaries for this rule.
Ipods, Cell Phones, and Other Devices.
Please note: If you decide to bring your ipod, cell phone, and/or any other expensive devices, ESUMC youth group and its leaders ARE NOT RESPONSIBLE if they are lost or stolen. You bring these items AT YOUR OWN RISK! Also, please DO NOT leave your money lying around your hotel room.
Banned Items
Do not bring any of the following items:
-FIREWORKS -DRUGS -ALCOHOL -TOBACCO PRODUCTS
-Movies rated higher than PG-13 -Games rated higher than T (Teen)
-ANY OTHER INAPPROPRIATE MATERIALS
If any of these items are found, they will be confiscated and may result in the participant(s) in question being sent home immediately.
