___Photos (3): one (1) Face portrait, one (1) full length, your designed clothing line

___Copy of your business Logo for MMDI website

___Photo size: (6" x 4"/15cm x 10cm).

__ Taiwan Expo. Fri, Sat. & Sun each day. One different outfit only per day

__ Sunday evening at the Appreciation/Gala/International Fashion Show; 4+different designed outfits for competition to win the Best Fashion Designer 2018

Important note:

Please email this application form, one (1) photo, a copy of your passport and proof of 100 Euro for registration fee and 8 days accommodation with three (3) meals per day by May 15, 2018. ALSO copy of your travel ticket with itinerary.

Also, MMDI needs your Medical Certificate showing you are healthy by your Doctor, It is a MANDOTARY.

If Pass May 15, 2018, late fee 150 Euro

If Pass June 15, 2018, late fee 200 Euro

After this application, MMDI official will send you a confirmation. After you get your confirmation.
