POSITION SUMMARY:

The Activity Based Recovery Programme Coordinator will provide goal and solution-centred community support that is underpinned by a philosophy of positively working with people accessing services to meet aspirations for increased health and general well being. You will provide recovery orientated activities and programmes to support Tangata Whaiora increase and reach their potential social and vocational aspirations. You will work collaboratively with Tui Ora Residential and Community Support team, mental health keyworkers and the Tangata Whaiora to develop activity plans that support recovery and community involvement. You will deliver services that will contribute to flexible Whanau Ora outcomes.

KEY RESPONSIBILITY AREAS:

Tāngata Whaiora/ Customer Service and/or Service Delivery

Key tasks:

§  Work collaboratively with Tangata Whaiora and Whanau to set and achieve the recovery goals of the Tangata Whaiora and support them to develop skills, access resources and develop independence in the community.

§  Develop individualised activity plans in partnership with Tangata Whaiora to increase daily occupational and vocational development and facilitate increased social interactions and activities.

§  Working within strengths based, recovery orientated mental health practice with Tangata Whaiora.

§  Contributing effectively to the provision of health education and support to Tangata Whaiora

§  Networking and liaising between both internal and external (to TOL) community supports to facilitate consumers ability to utilise local, accessible and affordable activity services.

§  Promoting the Tangata Whaiora’s needs and strengths so to develop/promote hope, self esteem, independent living skills, social skills and problem solving abilities.

§  Provide information and access to other community resources and services

§  Support and teach coping mechanism and strategies for managing mental health

§  Consider the Tangata Whaiora’s, mental, physical, spiritual and cultural needs

§  Promote the service user to become an active member of their own community connecting/reconnecting with people and activities

Ensuring the implementation of Whānau Ora model of care/ professional service by:

§  Clear and timely completion of all client notes and documentation

§  Ensuring every Tangata Whaiora has an individualized plan which reflects a holistic approach to their involvement with the service

Relationships

Maintain key partnerships by:

§  Building and maintaining relationships with providers to enhance multi-disciplinary service delivery, Tui Ora staff, other providers, Tāngata Whaiora/Whānau and stakeholders

§  Ensure an integrated approach to recovery planning with key workers and other stakeholders involved in the Tangata Whaiora’s recovery journey

§  Demonstrates self awareness and reflection

Communication/Team Work

Showing appropriate respect and understanding to each other by:

§  Contributing effectively and constructively to all communications throughout the organisation

§  Attending and actively participating in all staff meetings.

§  Write correspondence using appropriate language, grammar etc.

§  Utilise diverse methods of communication as tools for working with Tangata Whaiora

§  Ability to communicate appropriately without breeching the privacy or confidentiality of others

§  Confidently and competently operates computer systems

Ensuring the cultural needs of people using Tui Ora services and employees are met by:

§  Promoting and fostering Tikanga and Te Reo Māori

§  Maintaining a commitment to personal cultural development including Te Reo Māori and Tikanga Māori.

Ensuring Tui Oar’s quality framework approach is utilised at all times by:

§  Assisting with all service performance indicators/action plans to meet quality accreditation, legal and contractual obligations

§  Identifying service improvements, bringing them to the team meetings for discussion and assisting with the implementation of agreed plans

§  Ensuring all areas of concern/potential complaints are immediately brought to the attention of the Team Leader/ Manager

§  Completes reportable event forms as appropriate to ensure risks and quality issues are escalated

PERSON SPECIFICATION:

Qualifications/Education

§  Advocacy/Peer Support Qualification/Training preferred

§  Full, clean NZ Driver’s Licence

§  Commitment to on-going training.

Experience

§  Lived experience of mental health or addiction issues

§  Experience or is able to demonstrate an understanding of mental health and /or addiction services

§  Understanding of mental health, consumer and advocacy issues

§  Experience or demonstrate an understanding of reporting frameworks

§  A sound understanding of a recovery based philosophy

§  Experience of report writing and care planning

§  Has a working knowledge of Microsoft products/applications.

Competence

§  Understanding of Māori models of health and the application of the Treaty of Waitangi in practice

§  Ability to plan and achieve goals in an environment of scarce resources

§  Ability to communicate comprehensive information, including using presentations, written and orally

§  Proven ability to form collaborative/professional relationships with all stakeholder groups

§  Ability to think critically, coach and influence others

§  Practices, promotes and creates a culture that fosters high standards of ethics and integrity

§  Is personally trustworthy, delivers on promises and honours commitments

§  Ability to prioritise and make consistent recommendations based on fact and an accurate understanding of the situation

§  Able to demonstrate practical understanding of the aspirations and challenges facing Māori communities

§  Te Reo Māori and knowledge of Taranaki Tikanga and the ability to uphold and apply Tikanga Māori in the role is preferred/desirable.

Salary circa: $ range per annum pro rata dependent upon skills and experience
