Grades 6-8 Instructional Plan

Objective: Identify focus students AND focus standards to plan for targeted support over the next 3 quarters.

Background: The Acuity Assessment helps us identify our student’s strengths and next steps. The following table AND the Acuity Teacher Data Analysis sheet (Please make sure you save a copy in your individual name) allows us to reflect on Acuity Assessment trends. These documents will also guide the next Student Achievement Meetings (SAMs) at the end of November 2012. Have them ready to discuss.

The table below represents where the students sitting in front of us (AKA transition students) are in terms of proficiency. Transition students need to “maintain” proficiency, in other words we don’t want them to fall back. In addition, in order to meet our Unified Improvement Plan (UIP) Goals we need to move up a certain number (represented in table) by content area and grade level. Please fill in the table below as a team and complete your own individual Acuity Teacher Data Analysis sheet by the end of November to be fully prepared for your Q2 SAM’s meetings. We will focus our data driven dialogue with this data at the center of our conversation.

Once Acuity Assessment results have been analyzed, plans should be made for adapting instruction and helping students master the identified focus standards while not losing site of pacing guides/grade level expectations over the coming months. Summarize the plans your SALT team has made below:

Grade / Content Area / Focus students by teacher
(List the students that you will target for intervention) / Focus Standards
/Benchmarks
(# should exceed # of students who need to move up) / SALT Action Plans
