WORK PLACEMENT CHECKLIST

To be read in conjunction with work placement flow chart

STUDENT

Student has completed (or is in the process of completing) their OH & S module in their VET class before applying for any work placement.

Student has completed their Safe@Work OH& S work. This is completed in the home school and supervised and updated each year. Student must show this to their VET teacher.

Student finds their own work placement OR

Before the student completes an on line application for work placement through Western Local Community Partnership’s website, the Structured Workplace Learning MoU must be signed and training on how to use the website must be delivered by the teacher (student password to be created). Remember: students must allow 8 weeks for the application process.

Student gets DEECD work placement form (Ministerial Order 55) and organises Principal, parent and employer to sign form

  • At Mowbray if students go through Western LCP the student gives the acceptance form to Dianne Bouras immediately they receive them in the mail.In some cases she mails them to Employer for signature and then give copies to student to give to VET teachers.
  • A medical form may also be issued to students.

Student returns signed form to VET coordinator at their home school for checking and final confirmation. The original never leaves the home school.

Student receives a photocopy of work placement from their home school to give to their VET teacher

All paperwork should be competed prior to commencement of the placement.

VET TEACHER

VET teacher delivers O H & S modules

VET teacher monitors the workplacements in their class. If students have not found a placement they can

  • Seek clarification from Western LCP
  • Help the student find a placement
  • Ask the home school VET coordinator for some assistance
  • Postpone the placement until later in the year.

VET teacher reminds VET coordinator that work placements are due to begin (according to cluster calendar) and the names of students who should be going out

VET teacher emails all other VET coordinators and VCF as a courtesy to remind them work placements are starting

VET teacher collects photocopied work placement forms as they are returned with employer, principal and student signature – any originals are sent back the home school

VET teacher sends a summary list of who is/ is not participating to home schools and copies in the VCF; this must be done in the week prior to the placement not after the placement has started.

VET COORDINATORS

VET coordinators in each school notify all internal staff of up coming work placements to identify which students will be absent

VET coordinators in each school oversee all of the above process to ensure their VET teachers are following all correct policies and procedures.
