2018 North Carolina 4-H Youth Volunteer Award Descriptions
NC 4-H Youth Volunteer Awards are presented annually to individual teens that have provided exemplary service to their communities during the previous year. The application should highlight work done between May, 2017 and May, 2018. The program is designed specifically to recognize those 4-H’ers aged 14 -18 who excel in volunteerism and/or service to their communities.
State 4-H Youth Volunteer Recognition
Up to four state youth volunteer awards will be presented at the awards assembly during NC 4-H Congress. These awards will be presented to individuals who have demonstrated outstanding leadership and volunteer service during the past one year period. This is an individual award and is not intended to recognize clubs, teams or other groups of youth involved in volunteer or service work.
District 4-H Youth Volunteer Recognition
Up to two district award recipients will be named from each district for outstanding leadership and volunteer service during the past one year period. These awards will be presented to recipients at the awards assembly during NC 4-H Congress. This is an individual award and is not intended to recognize clubs, teams or other groups of youth involved in volunteer or service work.
NOTE: Youth may not be recognized as recipients in both district and state categories in the same year. Youth may be recognized in each category once during their 4-H career.

- Submit entire nomination (transmittal, nomination form, letters) as one file. It may be submitted as a Word or PDF document, but should be complete in one submission as one file. Applications which do not follow these rules will not be accepted.
- Nominations are incomplete if not signed by 4-H agent or designated employee of Extension.

Describe in 75 words or less the nominee’s volunteer activity in a statement suitable for publicity and for the program. Nomination is incomplete and may be disqualified if this section is not completed either in this space or on an attached page.

List major volunteer or community service efforts of the nominee. Include details as to level of involvement (did nominee plan the event, serve as chair of planning committee, etc.).
List any other recognition received for volunteer or community service efforts.
Describe the specific impacts of the volunteer or community service effort (numbers reached, dollars saved as a result, etc.)
Supporting Statements
Nomination should be accompanied by two supporting statements. These statements should be on separate pages and should include the following:
1. Agent or other volunteer’s statement of evidence of personal growth of youth being nominated.
2. Letter of reference from a community leader, 4-H’er or other volunteer.
