Assignment Guidelines: Behavior Theory Project

The Behavior Theory Project provides your group an opportunity to work on health counseling skills and to apply health behavior theory in developing a tailored program for an identified client. The group components described below are due at the beginning of class Tuesday, June 1, 2010.

NO LATE PROJECTS WILL BE ACCEPTED!

The following items are required for completion of this project.

1) The completed health behavior assessment handout (the individual worksheet) and a typed record of any follow-up questions you asked your client, followed by his/her response to each questions. This portion is due Thursday, April 22, 2010 at the beginning of class.

2) Identification of the theory you have been assigned to guide your program. A list of each construct of the theory followed by what information you have gathered from your client that is a potential positive or negative influence in that domain. For example:

Health Belief Theory

Perceived threat:

  • My client is minimally aware of the relationship between a sedentary lifestyle and increased risk of cardiovascular disease and some cancers.
  • My client feels goods right now and does not feel at risk for future health problems.
  • My client’s father has high blood pressure and early evidence of heart disease.
  • My client is unaware of the role of physical activity in managing stress.
  • My client is not overweight and equates scale weight to health.

3) Construction of a logic model that clearly demonstrates (pictures) your program and its theoretical underpinnings. Your logic model should include influential factors, assumptions, inputs, outputs, short-term outcomes (knowledge, attitudes, beliefs, skill, awareness) and medium-term outcomes (behavior change). Don’t forget to use arrows to demonstrate the relationships of the components in your model.
