VESTS / Two maintenance personnel were towing the Saab. As they only had 2 reflective safety vests and one was being used elsewhere, the maintenance staff was forced to contravene SD10-05. Only one of the two people moving the aircraft were wearing a reflective safety vest.
There are three tugs in CYXE as per Operational Directive 09-02 to ALL TWA employees: There must be a minimum of two persons to move an aircraft. As such, there should be a minimum of 6 reflective safety vests provided for CYXE maintenance staff to follow both SD10-05 and OD09-02. / C.A.P.--Director of Ground Operations will be purchasing more reflective safety vests for CYXE maintenance.

2011 / TRUCKS / Nothing has happened yet, however, an individual believes that our shop trucks should have a survival kit in them for traveling from base to base. The reason being is that an employee or anyone else could hit the ditch and then at least they would be a bit more prepared for it. Survival kit may include: shovel, tow ropes, first aid kit, ax, survival candle, reflective markers or cones, water, matches, blanket, booster cables. / As a company we have a maintenance program for each vehicle. We purchased first aid kits and reflective markers for the vehicles we operate on the public roads/highways. We have issued a memo, stressing the proper operation of our vehicles and being properly dressed or equipped with the proper clothing and safety equipment for the trip being taken. / 31

2011 / SF 34 / After TO out of YPA northbound, the FA opened the lavatory door to see that the cupboard that enclosed the removable lavatory had fallen open. The FA then noticed that the lavatory bucket was missing. The FA notified the Captain who instructed her to lock the door and not allow passengers to use it for the duration of the day.
On the ground in CYVC, maintenance from CYXE called to say that they had found the lavatory in the hangar, and reiterated to not allow passengers to use the lavatory until this bucket could be reinstalled in CYXE.
This could have been a very disgusting mess and a possible bio hazard. / C.A.P.--Spoke to ramp agents and confirmed that this is part of their checklist. They were not aware that this happened. They are now aware and will ensure that the bucket is on the aircraft in the future. If this happens again, it should be considered as having it part of the FA’s pre-flight checks. / 11
