Playful Paws Doggy Daycare
1. I understand that I am responsible for any harm caused by my dog while my dog is attending Playful Paws Doggy Daycare. I shall indemnify Playful Paws Doggy Daycare against any claims made against the partnership or losses or damages of any kind suffered by Playful Paws Doggy Daycare as a result of my failure to inform Playful Paws Doggy Daycare of any pre-existing condition my dog may have (such as illness or aggression problems). I understand and agree that in admitting my dog to Playful Paws Doggy Daycare, the facility has relied on my representation that my dog is in good health and has not harmed or shown aggressive or threatening behavior towards any person or any other dog.
2. I understand and agree that Playful Paws Doggy Daycare will not be liable for problems, damage, or injury caused by my dog provided reasonable care and precautions are followed by daycare staff. I understand that Playful Paws Doggy Daycare is fully insured. I release Playful Paws Doggy Daycare of any liability arising from my dog’s attendance and participation at the daycare. I understand that the daycare is a place where animals co-mingle in groups. I understand my animal may be placed in a crate for "time outs", rest periods, or if injured. I understand that when dogs play in groups, nicks and scratches may occur. Daycare staff may or may not notify me immediately. If the injury is not serious staff may choose to leave the dog until the end of the day and let me know about it when I pick up my dog. If the injury is serious I will be notified immediately.
3. I understand and agree that any problem or injury that develops with my dog will be treated as deemed best by Playful Paws Doggy Daycare. Either my veterinarian or Playful Paws’ veterinarian will be contacted in the event of a medical problem that is serious in nature. In the event that my dog becomes deceased I will be notified immediately of the situation and the body will be held at my request until my arrival, or taken to the veterinarian of my choosing.
4. I understand that if my dog is left at the daycare for a period of three days without contact from the owner this dog will be considered abandoned and necessary steps will be taken to turn the animal over to the proper authorities.
5. I, as the owner, agree to be solely responsible for any and all acts or behavior of my pet while in the care of Playful Paws Doggy Daycare. If my pet should become ill or seem to be in need of medical consideration, Playful Paws Doggy Daycare reserves the right to administer aid and/or to use any available veterinarian. I, the owner, shall pay any expenses so incurred.
6. In the event of inclement weather or natural disaster, Playful Paws Doggy Daycare is entrusted to use best judgement in caring for pets.
7. I understand that my dog is to be current in all vaccinations as required by my veterinarian and that a record will be requested to verify that status.
8. I understand that Playful Paws reserves that right to terminate attendance at any time.
9. I understand that images of my dog may be used by Playful Paws on their website, newsletter, advertisements or other promotional material.
