ELIGIBILITY SUMMARY ADDENDUM

FOR IDENTIFICATION OF SPEECH AND LANGUAGE IMPAIRMENT

DEFINITION

"Speech or language impairment" means a communication disorder, such as stuttering, impaired articulation, expressive or receptive language impairment, or voice impairment that adversely affects a child's educational performance. (34 CFR 300.8(c)(11))

NOTE: Children shall not be identified as children having a speech or language impairment if the area of concern is primarily the result of sociocultural dialect, delays/differences associated with acquisition of English as a second language, or within the purview of established norms for articulation and language development.

CRITERIA

I. The student has a communication disorder (i.e. stuttering, impaired articulation, Yes No
expressive or receptive language impairment, or voice impairment)
The committee must specify the communication disorder:

The child has a significant discrepancy from typical communication skills in one or
more of the following areas (one or more areas MUST be checked in order to respond “Yes”):
fluency
impaired articulation
expressive or receptive language impairment Yes No

The committee must justify the choice above with an explanation of the data and the data source:

II.  The area of concern is NOT the primary result of sociocultural dialect, True False
delays / differences associated with the acquisition of English as a second language.

If “False”, explain:

articulation and language development.

If “False”, explain:

IV.  There is an adverse effect on the child's educational performance due to one or Yes No
more documented characteristics of speech or language impairment;
The committee must describe the documented characteristic(s) of the impairment and
its specific and related adverse effect on educational performance below.

V.  Information from instruments that are culturally and linguistically appropriate, Yes No
including standardized and criterion-referenced measures, were used in conjunction
with information from classroom observations to determine the severity of the
communication impairment.

VI.  Result

If ALL of the above questions are answered “YES” / “TRUE”, this student meets the Federal, State and Local definition of this particular disability category. It is the “first prong” of the three step eligibility process.
