Making it Real Priorities and Action Plan
- I have care and support that is directed by me and responsive to my needs.

I have completed important to important for, and have a range of person centred tools in use in my preferred format. Personalised activity planner, staff boards and a weekly activity rota.
I regularly take an active role in 1-1s with my key worker.
I also take an active role in health and safety within my home, completing fire checks weekly, and ensuring my car and home are well maintained.
I work with staff to include new activities and ways of working, using PCP tools. / I have a limited understanding of the care support plans in place and therefore I am unable to work fully in partnership with support staff to make changes, although I am able to add input in some areas. / I have tried widget information on a daily basis – this can make me anxious as there is sometimes too much information and I become stressed / I have significant difficulties in taking part in meetings due to communication difficulties and anxieties, Therefore I need to have support within my home to communicate my wishes.
I need to understand the purpose of the care support plans in place to be able to makes choices and communicate any changes I may wish to make to how I am supported. / I take an active role in my 1-1s with support staff and am able to reflect on my week and communicate my wishes. I, with support, plan activities on a daily basis.
I complete a staff board and activity board daily with staff and have much of my information adapted to my communication needs.
I maintain regular contact with my family through letter writing and visits to my family home / Planning the information to be provided without overloading me and causing unnecessary anxieties.
I do not always have a voice in the way support is provided although staffs try to include me as much as possible. / Within the home staff to:

I have regular 1-1 meetings with my key worker, where we discuss any issues I have, what I have enjoyed, things I’d like to change, things I’d like to do, and who would help me achieve this.
I am able to take control of my day, every day and am able to decline activities or choose what activity I would like to do and where I might like to do this, like eating out for instance.
I also have information of names and addresses of the important people in my life and write to my parents each week. / I have a limited understanding of who else is available to support me to make things happen. / I decline to take part in reviews and PCP meetings.
Staff offer me choices within all aspects of my care. / I have significant difficulties in taking part in meetings.
Therefore staff need to work with me within my home for me to be comfortable in communicating my wishes. / I now take an active part in 1-1s and continue to have input into my days.
I also write letters to my family weekly. / The individual has limited understanding of who else could support them in making important decisions in their life. / Provide the individual with the information they need to be fully informed of who is available to make things happen, including contact details which should include email addresses alongside his family information. / Manager/Staff / To discuss in staff meetings

I have regular 1-1 meetings with my key worker, where we try to discuss any issues I have, what I have enjoyed, things I’d like to change, things I’d like to do, and who would help me achieve this.
I have completed important to important for, and have a range of person centred tools in use in my preferred format. Personalised activity planner, and staff board.
I take an active role in health and safety within my home, completing fire checks weekly, car checks and am involved in the upkeep of my home. / Much of my information is not in an accessible format meaning some of this information I am unable to access clearly.
I do not show an interest in accessing this information, although some information is available to me in my preferred format if I wish to access it. / Some of my information regarding my care has been converted into an easy read format using widget.
Again I show no interest in accessing this information but am able to if I wish.
Staff enable me to have my say and express my opinions in my regular 1-1 meetings and on an a daily basis / I tend not to show any interest in accessing my information.
I enjoy my 1-1 meetings and feel able to communicate my needs clearly.
Having my important to and important for information and my personalised tools has helped build friendships with staff and ensured my support is consistent. / Regular 1-1 meetings, communication tools and ability to express myself. / The individual does not actively participate in his records and information.
We continue to build upon the communication needs of the individual and continue to update PCP tools to ensure this. / Identify when things change for the individual and update information with their input when it does.
