Please fill out the following information, and submit this form to Maureen Gelwicks ( or fax 202-962-3939) no later than Friday, March 7th, 2014. Shipping instructions are below on page 2.

This form must be received in order to hold your spot at the bookstore. If you have multiple books, a form for each book is required. You will be notified ASAP if your book was not approved for sale in the Bookstore by the Bookstore Committee. Please note, you must be in attendance at the FIGT Annual Conference to have your book sold in the bookstore. Attendance will be confirmed prior to approval.

Book Shipping Instructions

Presenters are solely responsible for the shipping of their books. FIGT does not assume any responsibility for damaged, lost or stolen packages. Book packages can be shipped to the Tysons Corner Marriott. Packages to the hotel can be received no earlier than March 17 and no later than March 20, 2014 (this is the hotel’s policy).
