The Department of Health, Kauai District Health Office (KDHO), Public Health Preparedness (PHP) Program, is requesting a price quotation for the purchase of temporary staffing to support 1 project area related to public health preparedness.

  1. SERVICES

Description of services to be purchased:

  1. One (1) public health educator to provide mosquito-borne virus education to organizations and the general public.
  2. Must have a Master of Public Health (MPH), Master of Science in Public Health (MSPH) or equivalent degree.
  3. Must have at least two years of outreach/education experience.
  4. Must have some prior experience/background in mosquito-borne diseases and/or vector control efforts.
  5. Must be willing to work outside conducting mosquito elimination efforts.
  6. Must have extensive experience working with individuals from different cultures and backgrounds in low resource settings.
  7. Must have a personal laptop and be willing to set up a professional email account to communicate with local agencies.
  8. Must have significant experience in public speaking.
  9. Must possess excellent English skills (read, write and speak, fluency in other language a plus).
  10. Must have excellent Microsoft Office suite computer skills.
  11. Independent worker and able to take directions and complete tasks.
  12. Must have a personal car to use for travel around Kauai.
  13. Hours for the health educator shall averagenot more than 40 hours per week.

No modifications to any part of this solicitation by any party other than the State of Hawaii or its representatives are allowed. Any offer that contains any modification to any aspect or part of this solicitation will be deemed unacceptable.

  1. The KDHO PHP program reserves the right to deem the worker provided under this solicitation to be unacceptable within 7 days of initial meeting and job briefing.

II. SCHEDULE

The Department of Health, Kauai District Health Office (KDHO), Public Health Preparedness (PHP) Program operates during the hours of 7:45 A.M. to 4:30 P.M. Monday to Friday and is closed on the weekends and State holidays.

Please submit your price quotation by 4:00 pm, December 10, 2014on HIePRO. Award will be made to the lowest responsive, responsible offer. The offeror must be able to produce a Certificate of Vendor Compliance at the time of award and for final payment.

III. Criteria by which the “best value” to the state will be determined:

If all criteria mentioned above are met, the responsive, responsible offer will be determined by the lowest bid price for the unit

  • Bidders must be “Compliant” with Hawaii Compliance Express (HCE) at 4:00pm, “date of the award”, December 10, 2014.
  • Price should include all applicable taxes
