MITIGATING CIRCUMSTANCES FORM – GUIDANCE NOTES

Please read these guidelines before completing the front of this form.

DETAILS OF MITIGATING CIRCUMSTANCES

You should complete this form if there are matters or circumstances which have had a seriously adverse effect on your performance in any assessment. The circumstances should be current and have been unanticipated and beyond your control rather than disadvantageous conditions experienced throughout your studies (students with serious, long-term difficulties should contact Student Support Services).

If you need to continue on a separate sheet please mark it clearly with your student number and attach it securely to this form. In the very exceptional case of your wish for the full details of these circumstances to be kept confidential, you must enclose the details of the mitigation in a sealed envelope.

IMPORTANT: The following reasons ARE CONSIDERED AS ACCEPTABLE GROUNDS FOR MITIGATION

1 a serious personal illness which is not a permanent condition

2 the death, or serious illness, of a close family member, a friend or person for whom the student has a responsibility of care

3 sudden or unforeseen circumstances beyond the reasonable control of the student

The following reasons are NOT CONSIDERED ACCEPTABLE AS GROUNDS FOR MITIGATION:

1 family, work, financial or other general problems which lie outside of the circumstances identified above

2 poor awareness of the University’s Academic Regulations

3 being unaware of, or misunderstanding, a submission deadline or the date of an examination

4 computer, disc, printer or any other technical failure for which the student is responsible (students should ensure that they keep a back-up copy of their work).

SUPPORTING DOCUMENTARY EVIDENCE

You will need to provide details of any medical certificates or supporting documentation or statements from responsible third parties to explain the absence of/from any assessment. Medical certificates must be in the English Language or, if not, accompanied by a notarised translation, together with precise dates of illness specified. In the case of family bereavement, please provide the name, address and telephone number of a relative who would be willing to confirm the case outlined.

SUBMITTING THE FORM

This form must be submitted to a student adviser for an initial check of whether both criteria (as specified in Section 6 of the Academic Regulations) have been satisfied before the claim is formally considered.

All claims must be submitted by the following deadlines:

Examinations/Presentations 5 working days after the date of the examination/presentation
Other (eg: assignment) 5 working days after the official submission date (or agreed extension) of assessed work

IN EXCEPTIONAL CIRCUMSTANCES (e.g. when a student has been hospitalised), the form may be submitted by a Student Adviser OR Director of Studies on behalf of the student.

This form MUST BE SIGNED BY BOTH THE STUDENT AND STUDENT ADVISER or DIRECTOR OF STUDIES and returned to your local Assessments & Conferments Unit Office below:

Your claim will be considered by a properly constituted Faculty Mitigation Panel, making formal recommendations on the outcome of your claim to the Faculty Awards Board.
