Job Purpose
To work as part of a small team to provide one to one and group support to those clients accessing weight management services within the Solihull area. Effective management of client caseload to ensure timely, appropriate interventions and effective support of clients through the process employing behavior change techniques and signposting to partners and subcontractors.
Duties and Key Responsibilities
- Make initial contact and foster good, professional ongoing relationships with clients accessing weight management service.
- Support individuals to make sustained changes in behavior using brief, focused psychological interventions.
- Designing and implement health psychology baseline assessments of behaviour related to health outcomes.
- Evaluating the pattern of behaviour and formulating working hypotheses about the target behaviour based on the assessment.
- Clearly communicating findings and methods of supported change to clients.
- Working alongside triage staff and clinical dietician to provide comprehensive support to clients using the service
- Providing opportunity to access additional support through subcontracted services and partners within Solihull to include:- weight management services, physical activity providers, healthy cooking groups
- Evaluating and communicating the outcomes of health psychology interventions.
- Planning and prioritising of own workload to ensure appropriate levels of intervention are provided to each client
- Dealing within incoming calls and queries to the service and providing telephone assessments where required
- Providing support and guidance to triage, telephone based colleagues
- Completion and maintenance of client records and updating databases to provide key outcome and evidence monitoring of service
- Liaison with qualified health psychologist around queries and to ensure quality of delivery
- Signposting and referral to other agencies and services to promote sustained behaviour change and health outcomes
- Attendance at provider and sub contractor led sessions as required to support clients and assess quality of service delivery
- Design and completion of relevant paperwork to provide support to clients in the form of assessments, updates and action plans
- Feedback on caseload and sub-contractor performance to Line Manager as appropriate
- Promotion and marketing of the service to a range of professionals, community groups and partners in and around Solihull
Personal Development
- To actively participate in continued personal development through one to one and group sessions with Health Psychologist (SCCH Health Psychology Consulting)
- Participate in meetings, one to ones, appraisals and work based training as appropriate
General
- Report to Line Manager on workload, progress and results
- Manage own time and resources
- Prioritise workload in liaison with Line Manager
- Any other duties as agreed with Line Manager, commensurate to the post
