4-H Evaluation Sheet for County Project Medals
The following items are a list of criteria that are used to determine your success in your 4-H project. All criteria must be completed in a single project area to earn a medal in that project area. A 4-H member may only one project medal each year based on the County Record Book. A 4-H member may receive a medal more than once in a project area.
_____ Be enrolled in 4-H
_____ Complete 4-H Report Form for the first time
_____ Write 4-H Story (at least 1/2 page)
_____ Include project picture section (optional)
_____ Must be at least 9 years old when report is due
_____ May receive only one time
_____ Be enrolled, on the 4-H Enrollment Form, in the project you are reporting on.
_____ Complete all pages of the Project Report Form (pages 1 - 10). Picture section is optional.
_____ Participate in at least 8 club, county, district or state events during the year (page 2)
_____ List at least 5 learning experiences in your main project area reported on (page 3)
______List at least 5 new things learned in your main project area reported on (page 4)
____ Prepare at least 5 or more items for exhibit including the county fair exhibits (page 5)
_____ Present at least one speech or demonstration in your main project area reported on (page 6)
_____ Include income and expense information for your project, OPTIONAL for Bronze Level (page 7)
_____ Provide at least one form of leadership in any project area (page 8)
_____ Participate in at least one citizenship activity in any project area (page 9)
_____ Complete non 4-H experiences page (page 10)
_____ Write your 4-H Story.
_____ Project picture section (optional for Bronze Level)
_____ Be enrolled, on the 4-H Enrollment Form, in the project you are reporting on.
_____ Complete all pages of the Project Report Form (pages 1 – 10). Picture section is REQUIRED.
_____ Participate in at least 10 club, county, district or state events during the year (page 2)
_____ List at least 7 learning experiences in your main project area reported on (page 3)
_____ List at least 9 new things learned in your main project area reported on (page 4)
_____ Prepare at least 10 or more items for exhibit including the county fair exhibits (page 5)
_____ Present at least 4 public speaking presentations, 2 must be in your main project area reported on (page 6)
_____ Report 2 income and expense items for your project, one must be in your main project area reported on (page 7)
_____ Provide at least 4 leadership experiences to others, 2 must be in your main project area reported on (page 8)
_____ Participate in at least 3 citizenship activities, 2 must be in your main project area reported on (page 9)
_____ Complete non 4-H experiences page (page 10)
_____ Write your 4-H Story
_____ Include no more than 3 pages of pictures (one side only) with captions. Pictures should show you doing 4-H work, leadership and citizenship activities in your main project area reported on (no newspaper articles, certificates, or other types of documentation should be included).
_____ Be enrolled, on the 4-H Enrollment Form, in the project you are reporting on.
_____ Complete all pages of the Project Report Form (pages 1 – 10). Picture section is REQUIRED.
_____ Participate in at least 12 club, county, district, or state events during the year (pages 2)
_____ List at least 10 learning experiences in your project area reported on (pages 3)
_____ List at least 12 new things learned in your project are reported on (page 4)
_____ Prepare at least 12 exhibits including the county fair exhibits (page 5)
_____ Present at least 6 public speaking presentations, 3 must be in your main project area reported on (page 6)
_____ Report 4 income and expense items for your project, 2 must be in your main project area reported on (page 7)
_____ Provide at least 6 leadership experiences to others, 4 must be in your main project area reported on (page 8)
_____ Participate in at least 4 citizenship activities, 3 must be in your main project area reported on (page 9)
_____ Complete the non 4-H experiences page (page 10)
_____ Write your 4-H Story
Include no more than 3 pages of pictures (one side only) with captions. Pictures should show you doing 4-H work, leadership and citizenship activities in the main project area(s) reported on. (no newspaper articles, certificates, or other types of documentation should be included)
_____ Be enrolled in 4-H.
_____ Includes information on the project report form but does not meet the minimum
requirements for medal awards
_____ Write your 4-H Story.
